| Objective:To observe influence and curative effect of moving cupping on bladder meridian on lower limb spasm after stroke.Research method:Classify 96 sufferers randomly into moving cupping group, Baclofen group, acupuncture group according to principle of blind assignment principle,32 for each group. The acupuncture group:use conventional acupuncture and recovery training based on basic physical therapy; moving cupping group:move cupping on bladder meridian based on acupuncture group, starting from Dazhu point on back, move cupping continuously to Xialiao point on Lumbosacral region, then move cupping from Xialiao point to Dazhu point for one time, move cupping 30 times each time with speed of 10-20cm/s. Each treatment time is about 5min,1 time every other day,3 times every week, and 4 weeks for period of treatment; Baclofen group:take Baclofen orally on the base of acupuncture group. In the beginning, the oral dosing of Baclofen is 5mg every time,3 times every day, and the dosage is increased gradually within 4 weeks:increasing 5mg every three days till ideal effect is achieved and spasm is controlled, then maintain this dosage. Increase the dosage if the sufferers have drug resistance for current dosage. The maximal oral dosage can mount up to 80mg/day. Conduct the treatment of above three groups for period of treatment of 4 weeks. Assess and grade Ashworth, CSI, lower limb FMA and Holden walk function in the before treatment and first week, second week and fourth week after treatment.Research results:(1) Ashworth research results:There is no statistical significance (P>0.05)in Ashworth comparison among moving cupping group, Baclofen group and acupuncture group after 1 week and 2 weeks of treatment. There is statistical difference (P<0.05) in Ashworth comparison among the three groups in lower limb spasm after 4 weeks of treatment. Within four weeks after treatment, the Ashworth grade and time of moving cupping group and Baclofen group have trend of linear change (P<0.05); Ashworth grade and time of acupuncture of acupuncture group do not have trend of linear change (P>0.05).(2) CSI score research results:CSI scoring findings:for CSI scoring of the three groups, in multivariable check analysis, the time’s P<0.05, time* grouping’s P>0.05, and comparison difference (grouping) of the three groups’CSI scoring P>0.05. In CSI scoring comparison among the three groups at each time node, the comparison of scores for one-week treatment, two-week treatment and four-week treatment shows no statistical differences(P>0.05).(3) Research results of FMA score of spastic lower limbs:For the three groups of sufferers, in multivariable check analysis, time’s P<0.05, time*grouping’s P<0.05, and comparison differences (grouping) of lower limb FMA scoring for the three groups P>0.05. In comparison of scoring motion function of Fugl-Meyer lower limb for the three groups on each time node, P>0.05 for comparison between one-week treatment and two-week treatment, which has no statistical difference; P<0.05 for comparison after four-week treatment, which has statistical difference.(4) Holden walking function research results:In graded comparison of Holden walking function for the three groups, there are no statistical differences (P>0.05) for one-week treatment and two-week treatment. The comparison difference of Holden walking function after four-week treatment has statistical significance (P<0.05). Within four weeks after treatment, the Holden walking function scoring and time for the three groups all has trend of linear change(P<0.05). (5) After the end of the course of treatment, the moving Cupping group can improve the spasm of the upper limb and the strephenopodia were observed.By preliminary comparative analysis,In the curative effect comparations of the spasm of the upper limb and the strephenopodia.The moving Cupping group compare with baclofen group has no statistical significance(P>0.05);The moving Cupping group compare with acupuncture group and baclofen group compare with acupuncture group have statistical significance(P<0.05).(5)After period of treatment ends, it is observed that the moving cupping group saw improvement on upper limb spasm and strephenopodia. Preliminary comparative analysis shows no statistical difference (P>0.05) in comparison of curative effect between the moving cupping group and Baclofen group for upper limb spasm and lower limb spasm strephenopodia (P>0.05). There is statistical difference in comparison of curative effect for upper limb spasm and lower limb spasm strephenopodia between moving cupping group and acupuncture group, Baclofen group and acupuncture group (P<0.05).Conclusion:The urinary bladder meridian of foot after moving cupping therapy has obvious. |
