| BackgroundThe NCCN guidelines suggest people aged≥ 50 years should screen the colorectal cancer conventional. But several studies have showed that the colorectal cancer incidence rates of patients under 50 years old is on the rise, and the disease stage was relatively late.PurposeTo explore the clinical and pathological distribution differences in Young (< 50 y) and elderly (≥50 y) patients.MethodData were obtained from a retrospective database at zhejiang university affiliated Sir Run Run Shaw hospital from 2010 to 2012. There were 97 patients pathologically diagnosed with colorectal cancer under 50 years old,according to 1:2 proportion randomly selected 194 patients elder than 50 years old. Analyze the distribution differences about the time interval between symptoms to go to a doctor,symptoms,pathological pattern,stage of disease,3-year disease free survival rate ,3-year survival rate between two groups.ResultsThe time interval between symptoms to go to a doctor during two groups has no statistical difference (p=0.702).The younger have higher incidence rate of transverse colon tumor and double primary intestinal tumors than elder (p=0.050).The younger would more willing to accept chemotherapy (p=0.005).The proportion of mucinous adenocarcinoma in young patients (35.1%) is higher than elderly patients.Young patients had a higher proportion of well differentiated adenocarcinoma,while moderate differentiated is the most commom in the elder groups. The proportion of stage Ⅱ,Ⅲ tumors has significantly difference between two groups (p<0.001),in young adult group stage Ⅱ account for 36.4%,stageⅢ account for 35.1%;while the stage Ⅲ account for 44.9%,stage Ⅱ account for 30.7% in elder group.The 3-year disease free survival rate (77.9% vs 64.2% p=0.021) and 3-year survival rate (87% vs 83% p<0.001) showes that young adult group had a better prognosis.ConclusionThe time interval between symptoms to go to a doctor in the young adult group has no statistical difference with elder group.The stage of tumor in younger group is relative earlier than elder patients.through the analysis of 3-year disease free survival rate and 3-year survival rate,young adult group had a better outcome. |
