Objective:This study aims to explore the composition and change trend of colorectal cancer in different anatomical sites,and to compare the pathological characteristics of colorectal cancer in different anatomical sites and the clinical characteristics of patients,so as to provide theoretical basis for accurate diagnosis and individualized treatment for colorectal cancer patients.Methods:A total of 358 patients with colorectal cancer who received surgical treatment in the first hospital of qiqihar from 2009 to 2018 were studied,including 190 males,168females,47.8%of the elderly and 60.9%of the rural patients.By looking at cases and collect information,pathological types(adenocarcinoma and mucous adenocarcinoma and other types,etc.),degree of differentiation(high,medium and low or undifferentiated),clinical staging(???,?period),pathological characteristics,such as also recorded the age,sex,living habits,clinical symptoms of patients with clinical features.A total of 358 software was used for statistical analysis,and the composition ratio was used to express the gender,age group,pathological type and differentiation degree of colorectal cancer patients.The chi-square test and non-parametric rank sum test were used to compare the pathological types and clinical features of colorectal cancer at different anatomical sites in the three groups,and P<0.05 was considered as statistically significant difference.Results:1.From 2009 to 2018,the composition ratio of right colon cancer was higher than that of rectal cancer and left colon cancer,and the left colon cancer was the lowest.There was no significant change in the composition ratio of right colon cancer and left colon cancer during the 10 years,but the composition ratio of rectal cancer showed a gradual increase since 2011 to 2017.2.There were significant differences in the composition of the pathological types of colorectal cancer at different anatomical sites in the three groups(x~2=12.129,P<0.05).The cases of adenocarcinoma in the left colon cancer group accounted for the largest proportion,while the composition ratio of mucinous adenocarcinoma and other types in the rectal cancer group was higher than that in the other two groups.3.There were significant differences in clinical staging among the three groups of colorectal cancer patients with different anatomical sites(x~2=40.373,p<0.001).Left half colon cancer in patients with clinical stage for?cases constitute than higher than the right colon cancer and colorectal cancer group,the right of the colon and rectal cancer group stage?,?cases than group is higher than the left half colon cancer,rectal cancer clinical cases constitute?period than the highest in the three groups.4.There were significant differences in the differentiation degree of colorectal cancer at different anatomical sites in the three groups(Z=66.690,p<0.001).The proportion of highly differentiated cancers in the left half of the colon cancer group was the highest,higher than that in the right half of the colon cancer group and the rectal cancer group,while the proportion of low-differentiated and undifferentiated cancers in the right half of the colon cancer group was higher than that in the left half of the colon cancer group and the rectal cancer group.5.More men than women were found in the left and rectal cancers,and more women than men were found in the right colorectal cancers.The gender composition of colorectal cancers of different sites in the three groups showed statistically significant differences(x~2=16.534,p<0.001).Women with right-sided colon cancer had a higher composition ratio than those with left-sided colorectal cancer and rectal cancer,while men with rectal cancer had a higher composition ratio than those with left-sided colon cancer and right-sided colon cancer,respectively.6.There were significant differences in the composition ratio of age of onset of colorectal cancer in the three groups(x~2=37.905,p<0.001).The composition ratio of young people with rectal cancer was higher than that of left and right colon cancer,while the composition ratio of old people with left and right colon cancer was higher than that of rectal cancer.7.There were significant differences in the family history of colorectal cancer among the three groups of patients with different colorectal cancer sites(x~2=46.621,p<0.001).Those with a family history of colon cancer on the left side were more likely to have a family history of colon cancer on the right side and colon cancer on the rectum side.8.The presence of high-fat diet in patients with different colorectal cancer sites in the three groups was significantly different(x~2=28.877,p<0.001),and the proportion of high-fat diet in patients with rectal cancer was significantly higher than that in the other two groups.9.There were significant differences among the three groups in the presence of the following symptoms:weight loss(x~2=32.176,p<0.001),abdominal pain(x~2=52.236,p<0.001),abdominal distension(x~2=18.765,P<0.001),frequency of stool(x~2=12.775,p<0.001),blood stool(x~2=17.342 p<0.001),pus blood stool(x~2=24.296,p<0.001).Conclusion:1.The three anatomical positions of colorectal cancer from 2009 to 2018 were relatively balanced,but the proportion of colorectal cancer was on the rise from 2011to 2017,so early diagnosis and treatment should be paid attention to.2.The left colon cancer was characterized by high differentiation and early TNM stage,while the right colon cancer and rectal cancer had low differentiation and late TNM stage.3.Elderly people are prone to colorectal cancer,especially left and right colon cancer,left colon and rectal cancer patients were more male than female,people with a family history of colorectal cancer are more likely to develop left colon cancer.
