Objective In order to study the disease characteristics of young colorectal cancer,we analyzed its disease characteristics for strengthening the understanding of the disease,achieving early diagnosis and treatment of the disease,reducing the misdiagnosis rate,and ultimately improving the prognosis.Methods By summarizing relevant research results at home and abroad,this study conducted a retrospective analysis on 683 colorectal cancer patients admitted and treated in People's Hospital of Xinjiang Autonomous Region from October 2014 to September 2017,who met the inclusion and exclusion criteria.Clinical data and pathological data of patients who met the criteria were screened.According to patients'age,is divided into youth and old age,and the measurement data average±standard deviation is used to describe the data,Qualitative data were statistically described by case number and composition comparison,comparing differences between the groups with X~2 test,comprehensive analysis of young patients with colorectal cancer in elderly patients with colorectal cancer and the characteristics of the disease.Results(1)683 patients who met the study requirements were collected for clinical study.There were 141 young and middle-aged patients,with an average(35.51±3.91)years old,and 542elderly patients,with an average(68.33±5.72)years old.Family history of cancer:22 young patients and 47 old patients with family history.Misdiagnosis:55 young patients were misdiagnosed,and 123 elderly patients were misdiagnosed.Symptoms:among the young patients,38 had abdominal pain,39 had hematochelia,22 had no symptoms,32 had changed stool habits,and 10 had other symptoms.Among the elderly patients,178 had abdominal pain,138 had hematochezia,113 had no symptoms,51 had changed stool habits,and 62 had other symptoms.Tumor site:60 young patients were located in the rectum,52 in the left half colon and29 in the right half colon.The elderly patients were located in the rectum in 253 cases,left semicolon in 226 cases,right semicolon in 63 cases.Comparison of histological types between undifferentiated carcinoma and signet ring cell carcinoma in 21 young patients and 36 old patients.Lymph node metastasis:52 young patients and 131 old patients had lymph node metastasis.Distant metastasis:there were 20 young patients and 38 old patients with distant metastasis.TNM stage:young patients stage III 27 cases,stage IV 9 cases,elderly patients stage III 89 cases,stage IV 27 cases.Morphology of advanced colorectal cancer:there were 41 young patients with uplift type,14 with ulcer type and 37 with infiltration type.There were 203 cases of uplift type,66 cases of ulcer type and 13 cases of infiltration type.(2)inter-group difference comparison:the proportion of misdiagnosis in young patients,the proportion with a family history of cancer,the proportion with lymph node metastasis,the proportion with distant metastasis,the proportion of TNM stage III and stage IV was greater than that in elderly patients,and the difference was statistically significant.There were statistically significant differences in patients' symptoms,tumor sites and tumor histological types,but no statistically significant differences in patients'gender and tumor morphology at advanced stage.Conclusion Through the comparative analysis of the clinical and pathological data of young patients with colorectal cancer and elderly patients with colorectal cancer by statistics,the following conclusions can be drawn:?~2 test shows that young patients with colorectal cancer have the characteristics of high degree of malignancy,insignificant early symptoms,early metastasis,strong invasiveness and high misdiagnosis rate.To clarify the disease characteristics of young colorectal cancer,guide clinical work,increase the awareness of the disease,strengthen the degree of attention to the disease,improve the early diagnosis and treatment rate of young colorectal cancer,reduce the misdiagnosis rate of the disease,so as to improve the prognosis of young colorectal cancer patients,extend the survival time,improve the quality of life.
