ObjectiveTo summary 7 cases of adult Bartter syndrome and review the literature of Bartter syndrome to explore the etiology, pathogenesis, clinical manifestations, diagnosis, treatment and prognosis in order to have a earlier diagnosis and a more effective treatment in the future.MethodsClinical datas of 7 cases of adult Bartter syndrome admitted in the 1st affiliated hospital of Medieal College, Zhejiang University among 2011-2015 were analyzed retrospectively.ResultsThe 7 patients were all adult-onset, including six cases had varying degrees of fatigue, convulsions, palpitations symptoms, while one case had proteinuria. All patients showed hypokalemia, metabolic alkalosis, renin-angiotensin-aldosterone system activation with normal or low blood pressure. Serum potassium level was raised up and the symptoms relieved aftert reatment of potassium supplementation, spironolactone or indomethacin therapy.ConclusionAdult have fatigue, hypokalemia, metabolic alkalosis with normal or low blood pressure should be considered of adult Bartter syndrome. Clinical manifestations, laboratory joint standing and supine positions aldosterone basic test can substantially confirm the diagnosis. Treatments include potassium supplementation, spironolactone or indomethacin therapy and after effective treatments, serum potassium can keep nonnal for a long time. Generally, the prognos is good.
