Background of the researchPostoperative gastrointestinal functional recovery has always been a big problem after surgery. Gastric cancer and colorectal surgery is a major operation in clinical abdominal surgery, which making the Postoperative gastrointestinal functional recovery more important. Clinical medicine temporarily has not found effective methods to prevent the occurrence of postoperative bowel dysfunction. The only way to reduce the occurrence of postoperative bowel dysfunction so far, is to try to damage the intestinal tubes as less as possible during the surgery. Postoperative bowel dysfunction can lead to a variety of complications. First, Intestinal paralysis can cause slow intestinal peristalsis. And then long-term fasting will cause the intestinal mucosal barrier damaged, which can easily lead to bacterial translocation and cause serious infections, or worse, enterogenous sepsis. Besides, postoperative bowel dysfunction may cause difficulties in patients with postoperative wound healing. What’s more, slow Intestinal peristalsis can cause intestinal pneumatosis effusion, increasing the intestinal pressure, making anastomotic fistula possible. If anastomotic fistula breaks, secondary surgery will be unavoidable. Gastrointestinal fistula requires fast for a long time, which may possibly cause disturbance of water and electrolyte, or even acidosis and alkalosis, making the patient’s situation worse, increasing the time and expenses of hospitalization. All these consequences can cause unnecessary medical disputes and heavy psychological burden for both doctors and patients. According to the epidemiological investigation report, The incidence of gastrointestinal dysfunction after abdominal surgery is 15%-50%, therefore, attention should be paid to postoperative gastrointestinal dysfunction. Chinese medicine has its outstanding advantages in this field. Acupuncture on zusanli, outer coating with wuzhuyu, clysis with herb decoction are all effective for the recovery of gastrointestinal function, yet it still takes a long time for some patients to recover. Because of the tumor invasion, functions of the spleen and stomach of patients with gastric cancer, or colorectal cancer are always weakened. Old people are usually with pulmonary disease, obesity problem or smoking-related problem, which are all factors that lead to pulmonary ventilation dysfunction. Once it happens, low vital capacity and low oxygen content of tissues follow, making the driving force of promoting intestinal peristalsis of the powerless. TCM holds that after gastric cancer, colorectal surgery, the influence of operation and anesthesia will cause deficiency of both spleen and lung, which develops phlegm-damp. The main symptom is coughing and asthma usually with (high flow) oxygen uptake, which is exerts an important impact on patients’off-bed activities and the recovery of intestinal function. The purpose of this research is to reveal the advantages of TCM on the bowel function recovery after abdominal surgery, and try to explore a simple and convenient way to promote the recovery of gastrointestinal function after abdominal surgery.Purpose of the research To observe the recovery of gastrointestinal function after abdominal surgery with the help of acupuncture under methods of dispersing the lung and expelling wind, to study its mechanism of action, and to explore the application of TCM theory that is "the lung and the large intestine being interior-exteriorly related", and finally provide scientific theory basis for better recovery of gastrointestinal function after abdominal surgery.Methods of the researchTo observe the recovery of gastrointestinal function after abdominal surgery with the help of acupuncture under methods of dispersing the lung and expelling wind, we select patients that are under the situation of postoperative of gastrointestinal cancer while the differentiation of TCM is "qi and sputum stagnation" and divide them randomly into two groups, which are the conventional treatment group(30 patients) and the group under the treatment of dispersing the lung qi (30 patient). In the experimental group, we treat them with acupuncture under the method of dispersing the lung and expelling wind(mostly on lieque and chize acupoints), in addition to conventional western medicine treatment and observer their first time that fart and defecate after the surgery, hospitalization days, times that needed for them to resume normal diet, and the daily situation during their hospitalization. The experimental data will be analyzed with chi-square test, T test, and non-parametric test.research resultsWe divide patients randomly into two groups, which are the conventional treatment group(30 patients) and the group under the treatment of dispersing the lung qi (30 patient). In the experimental group, the average time they first fart after the surgery is 36 hours; the average time they first defecate after the surgery is 75.9 hours; and their average hospitalization time is 16 days. In the control group, results of the three dimension are 65 hours; 92.56 hours and 21 days. According to this result, we concluded that extra treatment with acupuncture under the method of dispersing the lung and expelling wind is statistically better than mere conventional western medicine treatment (P <0.05) on the postoperative recovery of gastrointestinal function.Conclusion of the researchAfter gastric cancer, colorectal surgery, the influence of operation and anesthesia will cause deficiency of both spleen and lung, which develops phlegm-damp. According to the TCM differentiation, with acupuncture under the method of dispersing the lung and expelling wind, the postoperative recovery of gastrointestinal function can be well promoted. It helps to relieve abdominal distension or pain symptoms; to accelerate farting and defecating; to develop the gastrointestinal function; to decline the chance of alteration of intestinal flora and ultimately servers the recovery of gastrointestinal function. Comparing two experimental data to each other, we can find that the average time patients first fart, defecate after the surgery and their average hospitalization time of the experimental group is obviously shorter than that in the control group. Therefore, we believe that acupuncture treatment which is under the method of dispersing the lung and expelling wind plays an important role in the postoperative recovery of gastrointestinal function.
