Research background:Lumbar disc herniation is a syndrome manifested because of fibrous ring disruption, stimulation or oppression of nerve roots and cauda equina by herniation of the nucleus pulposus; and a clinically common disease and frequently-occurring disease. Its morbidities of L4/L5 and L5/S1 are the highest. Its clinically classical symptoms are lumbago and backache and radiating pain of lower limbs. Patients are often aged between 25 and 55 and the number of male patients is greater than that of female patients.The treatment of lumbar disc herniation is divided into conservative treatment and surgical treatment. The conservative treatment is predominant.80-90% of patients are relieved or even cured by conservative treatment of traditional Chinese and western medicine such as acupuncture and moxibustion, Tuina, traditional Chinese medicine, physical therapy, and nerve block. In conservative treatment, acupuncture and moxibustion are widely used clinically due to their characteristics of simplicity, convenience, cheapness and efficiency. However, at present, efficacies of various conservative therapies on lumbar disc herniation are not satisfying with a yearly increased recurrence rate, so an optimization scheme for lumbar disc herniation treatment needs to be sought clinically. The round sharp needle was stood out due to its merits of speediness, effectivity and easy operation, but clinical reports on lumbar disc herniation treatment with round sharp needle were few. Thus, in the research, efficacies of round sharp needle therapy on treating lumbar disc herniation of different herniation types were observed, efficacies on the lateral type and the central type were compared, the therapeutic regimen of lumbar disc herniation was optimized for clinical guidance.Research objectives:Observation of clinical efficacies of round sharp needle therapy on treating lumbar disc herniation and efficacy comparison of different herniation types (including symptoms, pain degree, and improvement of living quality), further clarify the clinical indications in order to the purpose of clinical expansion and application.Research methods:1.70 patients with lumbar disc herniation who met the inclusion criteria were divided into two groups:a lateral-type group with 35 cases and a central-type group with 35 cases according to CT classification.2. Patients were of prone position. Jiaji points and Ashi points of their diseased regions and their upper and lower segmental lumbar were selected. With 75% alcohol after routine disinfection of skin, 0.8x50mm round sharp needles were used for treatment once daily by lifting and thrusting, perpendicular needling and proximal needling. After bringing about the desired sensation, the needles needed to be removed. Every 5 times constituted a treatment course.3. JOA, VAS and SF-36 scales were used for both groups and conditions before and after treatment were graded.4. The efficacy evaluation was made in line with evaluable standard on curative effects on lumbar disc herniation in "Clinical guideline of new drugs for traditional Chinese medicine" and "Standards for diagnosis and curative effect of Chinese medical symptom" formulated by State Administration of Traditional Chinese Medicine.5. Statistical analysis was conducted with SPSS 16.0 software, enumerationdata were expressed with % and the chi-square test was adopted for intergroup difference comparison; measurement data were expressed with, the independent-samples T test was adopted for intergroup difference comparison and the paired T test was used for comparison before and after treatment.Research results:1. The round sharp needle therapy was effective on both lateral-type and central-type lumbar disc herniation, but more suitable for patients with lateral-type lumbar disc herniation.2. Both clinical efficacies on both VAS and lateral-type were better than that of central-type (P<0.05).3. Comparative results of JOA and SF-36 showed that round sharp needle therapy was effective on both lateral-type and central-type lumbar disc herniation (P< 0.05), but the difference was not significant without statistical significance (P>0.05).Conclusion:The round sharp needle therapy was effective on both lateral-type and central-type lumbar disc herniation in respects of symptom improvement, alleviating pain, enhancement the life quality and clinical efficacy; and effects on the lateral type were better than those on the central type in alleviating pain and clinical efficacy.
