Objective:To observe the effects of modified Danxi gout prescription combined with knee arthroscopic debridement in treating blockage of phlegm and blood stasis type of knee gouty arthritis, and explore new therapeutic approach for knee gouty arthritis. Methods:Between November 2014 and June 2015, a total of 44 patients with blockage of phlegm and blood stasis type of knee gouty arthritis treated in Wendeng Orthopaedic Hospital of Shandong Province were enrolled in this study. The patients were randomly divided into treat group and control group according to random number table method. Knee arthroscopic debridement was performed on all patients. Modified Danxi gout prescription was administrated in test group on the first day after surgery; the control group was administered by gout treatment guidelines with lowering uric acid and non-steroidal anti-inflammatory drugs. Record the patient’s acute episode after surgery. Serum uric acid and subjective assessments include visual analog scale (VAS) pain score and Lysholm knee score before treatment (baseline) and at 1,7, and 14 days after operation were compared between two groups. Significant differences were determined with the Student’s t-test using SPSS 18.0 software. Results:Collected a total of 44 cases of patients included, of which 1 automatic discharge,1 case of self medication with drug interference,1 cases transferred out, total fall off in 3 cases, actual finish cases in 41 cases. There was no significant difference between the two groups of patients general condition including gender, age, disease progression and distribution of knees. Compared with before treatment, blood uric acid level, VAS pain scores of both groups of patients were significantly decreased after treatment (P<0.05), and Lysholm knee score and knee function were significantly improved (P<0.05). The effect of the test group was better than the control group in the prevention of acute attack of gout. There was no difference in lowering uric acid between treatment and control groups (P>0.05). Significant differences were observed in VAS scores and Lysholm knee score between two groups after treatment (P<0.05). After treatment, the treatment group knee function rating and improvement of symptoms and signs were superior in the control group (P<0.05).Conclusion:Modified Danxi gout prescription combined with knee arthroscopic debridement resulted in favorable clinical outcomes for the treatment of chronic gouty arthritis. It could significantly relieve the symptoms, lower uric acid levels with fewer side effects. Modified Danxi gout prescription could be clinically effective in the treatment of chronic gouty arthritis.
