Objective: Analysis of >75-year- old elderly people the main characteristic of intertrochanteric fracture, characteristics, to explore the use of relatively small incision closed reduction and internal fixation PFNA-Ⅱ bipolar hemiarthroplasty effect, to select more appropriate treatment options for patients.Methods: Treatment statistics from January 2014 to December 2015 during over 75 years of unstable intertrochanteric fracture of 34 cases. the patients were divided into A, B two surgical, A group of bipolar hemiarthroplasty, group B PFNA-Ⅱ fixation. Statistics incision length, operative time, blood loss, weight-bearing time, complications, and postoperative Harris score difference aspects.Results: Compared two groups,(1) the size of the incision B is better than A;(2) the length of time the operation B is better than A;(3) the amount of bleeding B is better than A;(4)The bed time A is better than B;(5) postoperative Harris score aspect A and B, two groups had no difference(p> 0.05).Conclusion: The elderly patients with unstable intertrochanteric fractures with osteoporosis, preoperative disease more comprehensive assessment by the individual choice of treatment is important. A small incision closed reduction and internal fixation PFNA-Ⅱor bipolar hemiarthroplasty treatment between various types of intertrochanteric fractures have a good effect. Hemiarthroplasty compared with PFNA-Ⅱ fixed bed within a short time. but PFNA-Ⅱhas smaller incision, less operation time, less trauma and blood loss.
