ObjectivesTo Explore the relationship between the shear wave velocity(SWV) with virtual touch tissue quantification(VTQ) and different syndromes of TCM in chronic kidney disease, and also to investigate the correlation between SWV and the chronic kidney disease stage. The study can provide objective and reliable evidence for diagnosis and treatment of the chronic kidney disease.MethodsBy prospective investigation from March 2015 to March 2016, Guang Dong Province Hospital Of Traditional Chinese Medicine Zhuhai branch of renal medical ward and outpatient service patients, who conforms to the nationa 1 Kidney foundation in 2002 in the organization of the Kidney dialysis/cl inical practice guidelines (K/DOQI) about CKD(Chronic Kidney diseases), an d while the 100 patients have the SWV value by using VTQ inspection of co lor Doppler ultrasound, also select 100 cases without CKD in the control group measure SWV value. According to the MDRD formula creatinine clearanc e the patients with CKD in accordance with the stage who can be divided i nto 5 groups to investigation SWV change between different grouping of CK D and the relationship between the case group and control group. Reference the Chinese medicine clinical research guidelines(2002 edition), the same person collect these patients of the clinical symptoms, and two subtropic al high above the doctors discriminate syndromes of TCM for these patient s. CKD can be divided into five kinds of TCM syndrome types, spleen and ki dney deficiency syndrome, spleen and kidney Yin deficiency syndrome, splee n and kidney Yang deficiency syndrome, liver and kidney Yin deficiency sy ndrome and Yin deficiency syndrome. The study is to investigate the distri bution of different period of TCM syndrome types in patients with CKD, the SWV values change in each type of syndrome and the relationship between TCM syndrome type and the SWV value change in patients with CKD periods.T hen according to the results of frequency statistics from the clinical da ta of patients with chronic kidney disease who also have the VTQ check,th e distribution of TCM syndrome, it is concluded that the correlation betwe en TCM syndrome and SWV value of patients with CKD. The combined CKD1-2 st age and CKD4-5 stage compared with the control group had no statistical d ifference(P>0.05).ResultsCKD stages can be divided into 5 groups,32 cases with CKD1 stage,2 st age of 23 cases of chronic kidney disease, CKD,31 cases of CKD stage 3.4 s tage of 5 cases of CKD,9 cases of CKD stage 5. Chronic kidney disease incl ude five kinds of TCM syndrome types,45 cases with spleen and kidney defi ciency syndrome, spleen and kidney Yin deficiency syndrome 21 cases,14 cas es of spleen kidney Yang deficiency syndrome, liver and kidney Yin deficie ncy syndrome and 10 cases of both Yin and Yang deficiency syndrome. SWV va lue of the case group and control group respectively (2.20±0.68)m/s、(2.38 ±0.84) m/s. Cases of SWV value lower than the control group, but there was no statistically significant difference is compared between two groups(P> 0.05). The stages of CKD is negatively related to the SWV value changes in trends, but has no significant difference(P>0.05), and each group respecti vely when compared with the control group had no statistical significance (P>0.05). The combined CKD1-2 stage and CKD4-5 stage respectively compared with the control group had no statistical difference (P>0.05).Butthe comb ined CKD3-5 stage respectively compared with the control group, CKD1 stage and CKD1-2 stage there were significant differences(P<0.05), GFR value co mparison between different CKD stage groups have obvious difference(P<0.0 5). Different CKD stage of TCM syndrome type distribution, and the distrib ution of TCM syndrome types in different CKD stage had no statistical sig nificance(P>0.05);SWV value between different TCM syndrome types of CKD g roups and GFR value comparison had no statistical significance(P>0.05), Ev ery syndrome type of traditional SWV value and GFR were compared between each group had no statistical significance(P>0.05), Different TCM syndrome types inCKD stages of distribution comparative differences had no statis tical significance(P> 0.05).ConclusionIn this research, VTQ technology can measure the kidney SWV value, SWV have negatively correlated with chronic kidney damage, but maybe cann’t re garded as a diagnostic index of case group and control group. SWV values c an be in a certain extent evaluation middle-late kidney damage, but cannot judge early kidney damage. Renal damage of CKD has no correlation with TC M syndrome type distribution. CKD syndrome, there was no significant corre lation with SWV values change with the change of TCM syndrome types,SWV v alue change with the change of TCM syndrome types is not obvious.Beside, with the damage is aggravating the TCM syndrome type of SWV value change is not obvious,not as a guide provide objective indicator of TCM syndrome types. This negative result may be related to distribution of cases in th is study.
