Objective:Children with primary Nephrotic Syndrome(Nephrotic Syndrome,NS)is a common chronic kidney disease in children,the incidence trend of rising year by year.Although similar clinical symptoms,treatment effect and prognosis of differences,most of the children after regular treatment to get better prognosis,but part of the case is not sensitive to treatment,becomes intractable NS,eventually lead to kidney failure,initial treatment in patients with such giving foot treatment or prolong the life time of the hormone can significantly improve the prognosis.The degree of renal interstitial lesions can be used as an important indicator to judge the curative effect and progress of NS children.Nephritic nephropathy is more important than simple nephropathy,and the renal interstitial lesions are more complicated with more severe glomerular lesions.The gold standard of renal interstitial lesions is renal biopsy,but it is difficult for children to carry out routine examinations,and the examination has certain traumatic risk,which limits its clinical application.Traditional ultrasound examination can only from the kidney internal echo and hemodynamic changes the size of the form to qualitative evaluation of kidney,not quantitative analysis of kidney tissue damage,a noninvasive clinical need Repeatability of testing means.Acoustic Touch tissue quantitative analysis(Virtual Touch tissue quantification,VTQ)technology is a new technology of ultrasonic elastography,by measuring the shear wave velocity of the renal parenchyma(shear wave velocity,SWV),to obtain corresponding parts of the elastic should be variable,evaluation of renal tissue elasticity changes,assessment of different clinical types of children with primary nephrotic syndrome in children to assess the diagnostic value of renal damage.Method:A total of 60 cases diagnosed with NS were divided into two groups,simple type-group and nephritic-group.At the same time,30 healthy subjects were selected as the control group.Virtual touch tissue quantification(VTQ)was used to detect renalSWV in all subjects.The results were compared and analyzed between groups.Results:Renal SWV between simple type-group and control group had no significant differences(P > 0.05).However,statistically significant differences were observed between nephritic-group and control group as well as simple type-group(F,7.19,P <0.05),The area under ROC curve diagnosised by virtual touch tissues quantification of simple type-group and nephritic-group primary nephrotic syndrome were 0.85 and0.95.Corresponding values of the best cut-off pointis were 2.56m/s and3.04m/s,sensitivies were 75.1%?92.9%,specificities were 80.9%?88.2%.Conclusion:The renal SWV of nephritic-group is obviously faster than simple type-group.So it plays an important role in the clinical classification and prognosis of nephrotic syndrome.it is very importent for clinicians to understand the state of illness and guide treatment and estimate the prognosis.
