Background and ObjectiveHeart failure is the terminal stage of all cardiovascular events, and is a major cause of morbidity and mortality. With the acceleration of aging process in our country, heart failure has become a major disease which threatens the health of aged population. Heart failure not only increased the governments’financial burden, but also caused great inconvenience and pain to patients and their families. Therefore, it’s essential to improve the prediction, diagnosis, and treatment of heart failure. Galectin-3 and sST2 can both reflect myocardial fibrosis, a key step in heart failure, and the level of sST2 and Galectin-3 are related to prognosis. However,there’s a lack of prospective randomized trials in patients under different cardiac function in hospital. This research aims to clarify the value of serological indicators in cardiac function and prognosis of heart failure study. The research method is evaluating the levels of sST2 and Galectin-3 in serum of the patients under different cardiac function classification.Methods1、191 patients with heart disease who received therapy in Shandong Provincial Hospital during April 2015 to January 2016 were chosen randomly. We completed the image and laboratory examination, and collected the peripheral venous blood, centrifuge for 10min at 3000r/min. The serum was packed in 0.5ml EP tube,-80 ℃ standby. Each patient received follow-up interview by clinics or telephone every 2 months.2S The levels of sST2 and Galectin-3 in sample were measured by ELISA kit(R & D,DST200、DGAL30).3、We analyzed the data using SPSS 22.0. The significance level we used was p< 0.05.Results1、The levels of sST2、 Galectin-3 increase from stage I to IV, according to the NYHA classification (p< 0.05).2、Patients with lower LVEF have higher levels of sST2、Galectin-3 (p=0.001). The levels of sST2 and Galectin-3 have no correlation with LVEDD (p=0.254).3、There are positive correlations between sST2 and Galectin-3, sST2 and NT-proBNP, Galectin-3 and NT-proBNP. Joint detection can optimize the accuracy and sensitivity of heart failure diagonsis.4、According to our research, the prognosis deteriorates as the level of sST2 increases(death, rehospitalization or recovering well at the present). The level of Galectin-3 is significantly high in patients with the worse prognosis.5、Accordig to our research, Galectin-3<15.67ng/ml、 sST2<31.74ng/ml can be a predictive factor of a favourable prognosis in the near future after discharge. sST2>45.031ng/ml can be a predictive factors of recent mortality. The patients with Galectin-3>21.90ng/ml and sST2>45.03ng/ml can have a shorter survival time.Conclusions1、The levels of serum sST2 and Galectin-3 and their interaction have positive correlation with cardiac function. The levels of sST2、galcetin-3 can reflect the cardiac function, and are independent of volume load.2、The level of sST2 can predict the prognosis of heart failure(death, rehospitalization or recovering well at the present). The level of Galectin-3 can predict the cardiac events in patients with heart failure. Galectin-3<15.67ng/ml or sST2<31.74ng/ml can be the predictive factors of a favourable prognosis in the near future after discharge. sST2>45.031ng/ml can be a predictive factors of recent mortality. The patients with Galectin-3> 21.90ng/ml and sST2> 45.03ng/ml can have a shorter survival time.3、Examining the levels of Galectin-3 and sST2 can be an important supplementary method to heart failure diagnosis and prognosis.
