Objective By copying IgA nephropathy rat model, To observe the effect of Shen Qi Di huang decoction on podocyte morphology in rats with IgA nephropathy and dynamic changes of expression of podocin, and to father explore the mechanism of how Shen Qi Di huang decoction reduces proteinuria of IgAN and try to find the approach to the therapy for IgAN through replication IgAN rats. Also it aims to provide more objective and scientific theoretical basis for assessing efficacy of traditional Chinese medicine therapy to IgA nephropathy.Methods 50 female SD rats were randomly divided into 5 groups, in addition to the normal control group, the other four groups were IgAN model. Rat IgAN model was induced by the improved method of oral feeding of bovine serum albumin(BSA) + carbon tetrachloride(CCL4) hypodermic injection + lipopolysaccharide(LSP) vena caudalis injection. After 9 weeks, successful Rat IgAN models were built,they were divided into IgAN model compared group(MC),controlled group of tripterygium (T), Shen Qi Dihuang decoction low-dose group(L), Shen Qi Dihuang decoction high-dose group(H). The controlled groups was given tripterygium, treatment groups are were given decoction of shenqi dihuang for next 8 weeks. N group and MC group were given distilled water.Results1. Successful Rat IgAN Model(1) Proteinuria was detected in IgAN model and had significant difference compared with the normal group (P<0.05);(2) The fluorescence intensity of IgA in IgAN model is strong positive, it means that we get the successful rat model.2. The Curative Effect Of Shen Qi Di Huang Decoction(1) The biochemical indicators:Quantitative urinary protein of 24 hours in T group, L group, H group were less than the MC group (P<0.05). Urinary protein levels had no statistical difference in N group and the three treatment groups (P>0.05). It had no significant difference in rats of serum creatinine blood urea nitrogen, serum albumin,glutamic-pyruvic-transaminase and glutamic-oxalacetic-transaminase in each group (P>0.05).(2) Renal pathology:The fluorescence intensity of IgA looked like branches thick and coarse granular in mesangial areas in MC group. Other groups of IgA deposition in mesangial areas were significantly reduced. MC group has manifestation of mildly and moderately mesangial matrix and cell proliferation. T group, S group, D group were improved compared with M group. The picture of Electron microscopy shows:the MC group has mesangial cells and matrix hyperplasia, Comparing with the N group, the fusion rate of foot process of MC significantly increased(P<0.05); the fusion rate of foot process in T group, L group, H group were significantly increased compared to MC group(P<0.05).(3) Podocin expression:Protein levels showed that podocin protein in MC group, T group, L group, H group were significantly decreased compared to N group(P<0.05). Podocin protein in T group, L group, H group were significantly increased compared to MC group(P<0.05), no significant difference was showned among the three groups (P>0.05). Expression of mRNA showed that the podocin mRNA of renal tissue in MC group was markedly lower than N group(P<0.05). The expression of podocin mRNA in T group, L group, H group were higher compared to MC group (P<0.05). There was no statistical difference between T group, L group and H group(P>0.05).Conclusion we get the successful rat IgAN model. Shen Qi Di huang decoction has the function of decreasing urinary protein, inhibiting mesangial cells decreasing the fusion rate of foot process and mesangial matrix proliferation, decrease the deposition of IgA in rats with IgAN model. It raises the expression of podocin protein and mRNA in rats. It hints that Shen Qi Dihuang decoction can reduce urinary protein excretion by raise the expression of podocin protein by regulating the expression of podocin, reduce the injury to podocyte and recover the damage of glomerular filtration membrane barrier.
