Objectives: To observe Yi Shen JiangTang decoction’s clinical effect on patients with type2diabetic nephropathy at the stage of CKD2,whose syndrome differentiation of TCM is both Qi-Yin deficiency with blood stasis. And to observe its effect on L-fatty acid binding protein (L-FABP) and Malondialdehyde (MDA).Thereby to evaluate the value of Yi Shen JiangTang decoction to DN of this type.And tried to explore the mechanism of Yi Shen JiangTang decoction in treating DN、oxidative stress state intervention blood vessels and delay the pharmacological mechanism of renal interstitial fibrosis based on the level of oxidative stress, provide new scientific basis for effective clinical practice of Yi Shen JiangTang decoction.Method:1. Sixty patients with DN were randomly divided into two groups,with30cases in each.2. The cases in control group were treated by simple western medicine which according to the Chinese type2diabetes prevention guide (2010edition) in the treatment in diabetic kidney disease. The cases in treatment group were treated by Yi Shen Jiang Tang decoction and western medicine.The treatment course is2month.3.Observe the changes of L-FABP、MDA、renal function、urine protein..Results:1.The level of L-FABP and MDA in the group of DN was higher than the group of health(p<0.01).2.HbAlc、FPG、2hPG and TG decreased in both groups after treatment(p<0.01),but there was no significant difference between them. eGFR increased in both group after treatment(p<0.01),but there was no significant difference between them.3.ACR、UAER、TC、LDL、L-FABP、MDA decreased in the two groups after treatment. And the treatment group is more effective than the control group(p<0.01).4.Bivariate correlation analysis:L-FABP and MDA are positively correlated (p<0.01).And they are positive correlated with ACR and UAER(p<0.01).Partial Correlation analysis:In the condition of limiting MDA,ACR and UAER is positive correlated with L-FABP(p<0.01); In the condition of limiting L-FABP, ACR and UAER is positive correlated with MDA(p<0.01);6.The total effective rate of the treatment group was90.0%,while the control group’s was73.3%from the view of TCM syndrome.There were significant differences between the two total effective rate of the treatment group was80%,while the control group’s was60%.There were significant differences between the two groups(p<0.01).Conclusion:1.L-FABP and MDA of patients with DN is higher than the healthy people, are positive correlated with ACR and UAER(p<0.01),Hint it may can be used as test indexes of early diagnosis and evaluation of a DN patients.2.L-FABP affect the DN progress prcess, L-FABP and MDA may exist in both the function of mutual promotion based on the level of oxidative stress.3.The therapeutic effect on ACR、UAER、TC、LDL of the treated group is better than that of the control group.4.Treatment with YiShenJiangTang decoction is more effective on patients with type2diabetic nephropathy at the stage of CKD2,whose syndrome differentiation of TCM is both Qi-Yin deficiency with blood stasis.And it is better than the treatment of simple western medicine.5. YiShenJiangTang decoction is effective on decreasing L-FABP and MDA. It may alleviate the vascular oxidative stress levels, improve endothelial cell function, thereby reducing the L-FABP and UAER, for there is a certain ease the DN’s development, deserves further research.
