ObjectiveThrough the study on health services utilization and equity of residents in Henan Province,we can understand the basic informationof health services utilization, and explore its influencing factors. In order to optimize the allocation of health resources, and put forward policy suggestions toimprove the residents’ health service fairness.MethodsOn the basis of literature analysis, collecting data from the fifth national health services survey in Henan Province and the allocation of health resources. The statistical description, Chi-square test and Logistic Regression were conducted on the raw data, to evaluate the demand and utilization of health services in Henan Province.From the perspective of population and economic distribution, usingGini coefficient, Lorenz curveto analyze the status and equity of health resources allocation in Henan Province.The concentration index(CI) ofhealth service utilization was calculated with quintile method to evaluate the equity.Data was double entried by Epidata3.1 and processed by R software.Results1.Health services needs of residents:Based upon the review of Fifth Health Services Survey in Henan, the two-week prevalence rate of residents over 15 years inHenan Provence is20.81%,among them, the two-week prevalence rate of residents over 60 years was the highest, 44.56%.Thatrate of female was significantly higher than that of the male(?2=15.524,P<0.001). The two-week prevalence rate of resident’s literacy in primary school and below was significantly higher than that in other educational levels(?2=787.681,P<0.001).The prevalence rate of chronic diseases was 24.25%,the top three of chronic disease prevalence rates were respectively high blood pressure, diabetes, heart disease and chronic diseases prevalence rates were significantly different among different gender, age group, marital status, educational levels, employment status,income groups, medical insurance types.2.Residents’ health services utilization:Thetwo-week consultation of residents over 15 years old was 2269, multi-factors Logistic Regression analysis indicated that age, marital status, medical insurance types, annual per capita incomes were the two-week consultation’s influence factors; in the recent year, there were 3193 residents who should be in hospital, and there were 688 residents who should have been in hospital, accounting for 21.55%; the influence factors of in-patient services were age and annual per capita incomes.3.Distribution of health resource allocation in Henan Province:According to the population distribution, the Gene coefficient of medical and health institutions, hospital beds, health technicians, physicians, and nurses in Henan Province health resources were 0.149, 0.188, 0.130, 0.124, and 0.189.According to the economic distribution, the Gene coefficient of those were 0.287, 0.108, 0.159, 0.132 and 0.109.4.Equity of residents’ health services utilization in Henan Province: The Concentration Index of the two-week non-consultation was 0.020, the Concentration Index of residents who should have been in hospital was-0.052; the Concentration Index of the two-week prevalence rate and chronic disease prevalence were respectively-0.056 and-0.093.Conclusions1. Health services need of residents with different income levels was different, and it showed a trend that there is high demand while low utilization in Henan Province. Compared with high-income groups, low-income people need more health services.2.The integrated equity of health services utilization of residents in Henan Province was weak. Main factor affecting fairness is economic income. The government should pay more attention to the low-income people with chronic diseases.3.The whole fairness of the allocation of health resources in Henan Province was good, the equity of population distributionwas better than that of economic distribution; the government should strengthen the allocation of health resources in areas with weaker economic levels, and improve the equity of health resources allocation further.
