| Objectives To investigate the status of stigma,family function,coping modes and posttraumatic growth in patients with permanent stoma;to analyze the fluencing factors of posttraumatic growth in patients with permanent stoma;to analyze of the correlation between stigma,family function,coping modes and posttraumatic growth in patients with permanent stoma;to explore whether the family function and coping modes of patients with permanent stoma play an intermediray role between stigma and posttraumatic growth.Methods Convenient sampling method was used to select patients who with permannent stoma after colorectal surgery from July to December in the stoma clinic in a third-grade class-A hospital in Tianjin.The General Information questionnaire,Social Impact Scale,Medical Coping Style Questionnaire,and Chinese-Posttraumatic growth inventory were used to investigate the 419 patients with permanent stoma.SPSS21.0 software were used to analyze the data.Descriptive analysis,t test,one-way ANOVA,correlation analysis,multiple stepwise regression analysis were included.The Process program was used to analyze the mediation effect.Results 1.The stigma score of 419 permanent stoma patients was(59.49 ± 12.52),at a moderate level;family function score was(6.40 ± 2.02),at a moderate level;for coping modes,facing dimention score was(17.74±5.31),lower than norms(P<0.01);avoiding score was(15.44±4.43)and yielding score was(11.16±3.60),higher than norms(P<0.01);posttraumatic growth score was(54.46 ± 12.47),at a moderate level.2.The results of t test and one-way ANOVA analysis showed that there were statistically significant differences in age,marital status,household companion,home care ostomy pattern,educational attainment,ways of medical payments,work statue,monthly income of family,cost of stoma care products per month,postoperative time of stoma,and the presence or absence of stoma complications(P<0.05);there was no significant difference in gender,ethnicity,religion,place of residence,and daily outings(P>0.05).3.Correlation analysis results showed that stigma was negatively related to posttraumatic growth,family function,face,and positively related to avoidance and yielding;posttraumatic growth was positively related to family function and face,and negatively related to avoidance.4.The results of regression analysis showed that stigma was a hindrance to posttraumatic growth;family function,face,self-care,having a spouse,no complications,and high education were the factors that promote posttraumatic growth.5.The analysis of the mediating effect showed that family function and coping modes were mediating variables that affect the posttraumatic growth of stigma.Conclusions 1.The stigma,family function,and posttraumatic growth of intestinal stoma patients are at medium level,patients are prone to take the coping mode of avoiding and yielding.2.Different demographic variables and disease-related characteristics have certain effects on posttraumatic growth of patients with colostomy,clinic should combine with the actual situation of the patient to implement targeted intervention measures.3.The lower the stigma and the higher the level of family function in patients with intestinal stoma,the more coping styles they have to deal with,and the higher the level of posttraumatic growth;the sense of stigma,family function,coping modes,home care ostomy model,marital status,presence or absence of ostomy complications,and education level are the factors that influence posttraumatic growth.4.Stigma can directly affect posttraumatic growth,and can also indirectly affect posttraumatic growth through family function and coping modes,family functions and coping modes play a mediating effect between stigma and posttraumatic growth. |
