Objective1. To evaluate the long-term clinical results and patient satisfaction of implant-retained/supported prostheses in edentulous patient based on retrospectively summary of the clinical data of 62 implant-retained/supported prostheses patients.2. To compare the occlusal parameter of 12 implant-supported fixed denture occlusal and 12 natural dentition by computerized occlusal analysis system. Discusses occlusal characteristics with implant supported prostheses in edentulate patients.Methods1. Between March 2005 to June 2015,304 implants were placed in 84 edentulous patients (110 jaws) with implant-retained/supported prostheses in Stomatology Hospital of Shandong University. The patients are in good health, without implanting operation contraindication. Follow-up to 62 cases (68 single jaw), a total of 242 implants.22 cases were lost follow-up, rate of the defaulters was 26.2%. The reason for lost follow-up are original medical records lost and disconnect from the patients. Additional Surgeries obtains maxillary sinus augmentation, localized management of sinus floor, alveolar bone distraction, osteotome technique and so on. Repairment methord with fixed denture (sectional fixed bridge in 11 cases, one fixed bridge in 3 cases) or overdenture (telescope crown 8 cases, clip-rod in 2 cases,13 cases of magnetic attachment,28 cases improved telescope crown+magnetic attachment, clip-rod+magnetic attachment 1 case, ball-cap in 1 case,1 Locator). To evaluate the denture with soft tissue health, clinical examination, X-ray observation, patient satisfaction, etc.2.Between 2012 to 2015,12 edentulous patients with implant-supported prostheses and completed their repairment for more than six months in Stomatology Hospital of Shandong University and 12 natural dentition patients by using Teetester occlusion system. Measuring the occlusal force, distribution and the time character of normal occlusion at the intercuspal position, the protrusionposition, left and right lateral position.Collecting data, and using the SPSS for descriptive and comparative statistical analysis to obtain the corresponding results.Results1. A total of 242 implants in 62 cases (68 single jaw). There are 38 male and 30 female, and the average age was 63.7 ± 11.5 years, average follow-up time was 40.44 months, average physician office visit times was 8.11. Implant failure was 9, implants cumulative survival rate of implant-supported overdenture of 3 year is 93.84%,5 year is 92.09%, the cumulative survival rate of implant-supported fixed denture is 100%. Peri-implantitis in 11 cases, the mPLI of implant-supported overdenture was 1.058±0.935, mSBI was 0.442±0.612, PD was 2.202±1.104, the mPLI of implant-supported fixed denture was 0.98±0.535, mSBI was 0.276±0.402, PD was 1.424±1.228.16.18%(1/68) cases without additional surgery; sinus lift, GBR, bone condensing and bone grafting were the most in additional surgery. Maxillary sinus lift 11 cases, accounting for 44%(11/25) in maxillary, GBR 17 cases (17/68, 25%), bone condensing 16 cases,22.06%(16/68). Implant-supported overdenture of 82.4%(56/68), telescope crown + magnetic attachment 51.9%(28/54), telescope crown 51.9%(8/54) and magnetic attachment 4.1%(13/54) is the most commonly used methord in this case. Sectional fixed bridge 78.6%(11/14), and one 21.4%(3/14). Mucosa after restoration, and the upper structure complications, a total of 39 cases (57.4%), with the telescope crown 50%(4/8),69.2%(9/13) magnetic attachment, modified telescope crown +magnetic attachment 71.4%(20/28),42.9%(6/14) fixed bridge. Fixed restoration repair frequency is 1.4 times on average, overdenture repair rate 1.43 times. Patient satisfaction with an average of 6.484 ±4.991 points in implant-support overdenture,3.226±2.845 points in implant-support fixed denture.2. The OT value and the biting force exised significant difference in intercuspal occlusal. The DT value exised significant difference in protrusion occlusal. The DT value and occlusal contact exised significant difference in left and right lateral occlusal.Conclusion1. The success rate of repairment with implant-retained/supported prostheses in edentulous patients is high.2. The implant failure rate and peri-implantitis incidence is lower in edentulous patients with implant-supported fixed denture.3. The restoration complications is higher in edentulous patients with implant-support overdenture.4. Patients with higher overall satisfaction with implant-retained/supported prostheses.5. The occlusion time, disclusion time, left and right side bite force balance can indicate the bite normal or not in clinical.6. At protrusion occlusal, the occlusal force are concentrated in the front teeth, the force percentage are heavier, but the occlusion pressure are smaller. 7. At left and right lateral occlusal, the occlusal are diverse in implant-supported fixed denture occlusal and natural occlusal, but the most was the canine protection occlusion and the functional occlusion.
