Objective To explore the risk factors on patients complicated pulmonary hypertension (PH) which is secondary to maintain hemodialysis.Methods We conducted a retrospective study of 98 patients on maintain hemodialysis that treated in Nephrology Department of First Affiliated Hospital of Chongqing Medical University during November 2011 to May 2014. According to the results of echocardiography examination, patients are divided into two groups:those with PH and those without PH. Age, gender, type of vascular access, primary diseases, laboratorial and echocardiography findings are all compared between the groups. Using SPSS 17.0 statistical software analyses of the above factors, to explore the risk factors for patients with PH complicated by end-stage renal disease.Results In a univariate analysis, Diastolic blood pressure before dialysis, LnPTH, LnBNP are increased significant in PH group. Left ventricular diastolic diameter, Left ventricular systolic diameter, Interventricular septum thickness, left ventricular wall thickness,left atrial diameter, right ventricular diastolic diameter and right atrial diameter are also became bigger in PH group. On the contrary, Hb, Fraction shortening (FS), Left ventricular ejection fraction (EF), Ventricular septal pulsation amplitude, left ventricular posterior wall pulsation amplitude are decreased significantly. The Logistic regression analysis showed that the related factors of MHD complicated with PH are the natural logarithm of PTH(LnPTH), the natural logarithm of BNP(LnBNP), EF, Hb, Interventricular septum thickness, Left ventricular systolic diameter. And the Logistic regression equation is:y=15.528+0.332x1+0.928x2-0.340x3-0.049x4+0.653x5-0.219x6. (xi=LnBNP, X2=LnPTH, X3=EF, X4=Hb, X5=Interventricular septum thickness, X6=Left ventricular systolic diameter).Conclusion It is common in patients of MHD complicated with pulmonary hypertension, and primary diseases of end-stage renal disease has nothing to do with it. The groups differed in Diastolic blood pressure before dialysis, Hb, LnPTH, LnBNP, FS, EF, Ventricular septal pulsation amplitude, left ventricular posterior wall pulsation amplitude, Left ventricular diastolic diameter, Left ventricular systolic diameter, Interventricular septum thickness, left ventricular wall thickness, left atrial diameter, right ventricular diastolic diameter, right atrial diameter. According to Logistic regression analysis, LnPTH, LnBNP and Interventricular septum thickness are independent risk factors for patients who suffered MHD complicated PH. On the contrary, increasing EF and Hb, decreasing Left ventricular systolic diameter are protecting them from PH.
