Objective: To expore the risk factors and causes of pulmonary embolism in Chronic obstructive pulmonary disease.And to apply it in the clinical for the early diagnosis and intervention,so as to improve the quality of COPD patients' life and reduce its mortality.Methods: Retrospective analysis of 3660 COPD cases from March 2004 to September 2015 in the Second Hospital of Hebei Medical University hospital.Patients were divide into two groups according to whether with pulmonary embolism or not.We used statistical software spss22.0 to analyse the of clinical data of two groups.Wilcoxon rank sum test,chi-square test for univariate analysis and Logistic Regression Model analysis were used.Significance level ?= 0.05.Results: Univariate analysis showed: gender,pulmonary hypertension,heart failure,deep vein thrombosis and platelet?PLT?,creatine kinase?CK?,creatine kinase?CK-MB?,D-dimer?D-dimer?and artery carbon dioxide pressure?PCO2?and oxygen pressure?PO2?can affect the incidence of pulmonary embolism in COPD?P<0.05?.Multivariate Logistic regression analysis of significant variables in the Univariate analysis showed that deep vein thrombosis,pulmonary hypertension,blood pressure?<60 mmHg?,gender were risk factors for the incidence of pulmonary embolism in chronic obstructive pulmonary disease.Sort according to the degree of risk from high to low as follows: deep vein thrombosis> Pulmonary Hypertension> PaO2?<60 mmHg?> gender.?OR=8.318,3.987,1.774and1.762?.Conclusion: Chronic obstructive pulmonary disease is at high risk of pulmonary embolism,pulmonary embolism is due to the high mortality in patients with COPD.Factors affecting the occurrence of COPD and pulmonary embolism are manifold:1 age,occupation,history of chronic obstructive pulmonary disease and the frequency of its seizures can not be used as a basis to predict COPD pulmonary embolism.2 Gender,pulmonary hypertension,heart failure,deep vein thrombosis and platelet?PLT?,creatine kinase?CK?,creatine kinase?CK-MB?,D-dimer?D-dimer?artery blood carbon dioxide partial pressure?PCO2?and oxygen partial pressure?PO2?are the influencing factors of chronic obstructive pulmonary disease and pulmonary embolism.3 Multivariate Logistic regression analysis showed that: deep vein thrombosis,pulmonary hypertension,the reduction of blood pressure,and sex are relatively independent risk factors for the incidence of pulmonary embolism in chronic obstructive pulmonary disease.The occurrence and development of pulmonary embolism COPD complicated with pulmonary hypertension are reciprocal causation.When COPD patients with the above risk factors,further examination should be taken to clarify a dianosis,such as CTPA.
