Objective To investigate the pathogen distribution and antibiotic sensitivity in community-acquired pneumonia for the elderly,in order to support for clinical treatment and scientific use of the antibacterial drugs.Methods During 2012 to 2014,165 cases of community-acquired pneumonia in the elderly with the results of sputum culture were analyzed retrospectively.Results Totally 112 strains of bacteria were isolated, Gram-negative bacilli 82 strains (73.2%), Gram-positive bacilli 26 strains (23.2%), Fungi 4 strains (3.6%), of which the leading five bacteria were 28 strains of Klebsiella pneumoniae (25%),17 strains of Escherichia coli (15.1%),12 strains of Pseudomonas aeruginosa (10.7%),11 strains of Streptococcus pneumoniae (9.8%),10 strains of Haemophilus influenzae (8.9%). The results of drug sensitivity test showed that:Klebsiella pneumoniae strains to imipenem cefoperazone/sulbactam piperacillin/tazobactam susceptibility rate of 100%.Levofloxacin,ceftazidime,cefepime, ciprofloxacin,aztreonam,amikacin remain high sensitivity(more than 80%). But to ampicillin, cefuroxime resistance rate of more than 70%. Cefoperazone and piperacillin combined beta lactamase inhibitor compound preparation sensitivity increased significantly than single use. Escherichia coli are sensitive to imipenem,amikacin,piperacillin/ tazobactam with high sensitivity(more than 80%). ampicillin, cefuroxime, cefotaxime, cefepime, ceftazidime, levofloxacin, gentamicin, ciprofloxacin,cotrimoxazole, aztreonam resistance rate of more than 50%. The sensitive rate of Pseudomonas aeruginosa to imipenem, ciprofloxacin, levofloxacin, Amikacin reached 100%, To ceftazidime, Cefoperazone/ Shubatan, gentamicin, cefepime, piperacillin/tazobactam have high sensitivity(more than 80%), high resistance rate to minocycline, cotrimoxazole. Streptococcus pneumoniae are sensitive to vancomycin, Amoxicillin/clavulanate potassium, clindamycin with high sensitivity(more than 80%), sensitivity to macrolides about 63%, high resistance rate to penicillin, cefuroxime. Staphylococcus aureus are sensitive to vancomycin, Amikacin, teicoplanin, levofloxacin with high sensitivity(more than 80%), high resistance rate to azithromycin, erythromycin, clindamycin, oxacillin, all resistant to penicillin.Conclusion Gram-negative bacterium was the main pathogenic bacte--ria in community-acquired pneumonia in the elderly, which was more appropriate for the empirical selection of antibiotics with beta lactam/beta lactamase inhibitor compound preparation.
