Objective:1. Through investigation and study on the catheter-complications of external jugular vein type to explicit complications rate of each external jugular vein type.2. Through analyze the difference of the complications rate and kinds between external jugular vein types, to reveal the relevance between external jugular vein type and catheter-related complications.3.Through discuss the relevance between external jugular vein type and catheter-related complications,provide clinical nurses clinical reference for the vascular assessment before the placing catheter.And provide clinical gist of placing catheter via external jugular vein to filtrate vein, and to guide clinical nursing staff corresponding nursing measures according to different external jugular vein type. Methods:This study adopts the method of random surveys, select 159 tumor patients from September 2014 to December2015 hematology department of a top three hospitals in Changchun who with external jugular vein venipuncture as the research object.Every patients with catheter were determined the external jugular vein types, blood vessel diameter and blood flow velocity, lining whether smooth under ultrasound. After catheter in patients was checked by Radiographic to ensure the top position, followed up for 1 month.Use Excel to establish database and SPSS17.0 to data processing and statistical analysis, normal distribution data with mean± standard deviation( x ± S)or the number of patients and the percentage, enumeration data used Chi-square Test to analyze, The data are analyzed by t Test. Results:1.159 patients with external jugular venipuncture, external jugular vein under ultrasonic classification: type I :39cases, type II :50cases, type III:70cases. 159 patients with external jugular venipuncture, one-time successful:155 cases of catheter insertion, one-time success rate of catheter was 97.5%.159 patients with external jugular venipuncture, 29 cases of complications, the rate is18.2%. Including 11 cases of primary catheter heterotopia, secondary catheter heterotopia 5 cases, 7cases catheter-related phlebitis,5catheter-related thrombosis, 1 cases catheter fracture.2.39 cases of external jugular venous catheter in patients with type I succeeded in catheter 38 cases, one-time success rate of catheter was 97.4%;4cases with complications, including 2 cases of primary catheter heterotopia, 2 secondary catheter heterotopia.50 cases of external jugular vein in patients with type II catheter one-time catheter successfully in 49 cases, catheter success rate is 98.0%;2cases of complications, including primary catheter heterotopia 1 cases, 1cases of catheter-related phlebitis. 70 cases of external carotid venous catheter in patients with type III, one-time catheter successfully in 68 cases, one-time catheter success was 97.1%;20 cases of complications, including 8 cases of primary catheter heterotopia, secondary catheter heterotopia in 3 cases, 6 cases catheter-related phlebitis, 5 cases of catheter-related thrombosis,1 cases catheter fracture.3. The different external jugular venous complications difference between types has statistical significance(P < 0.01).External jugular vein type III complication rates higher than the incidence of complications of type I and type II, are not that different from external jugular vein type I complication rates and the complication rate of type II.4.The difference of primary catheter heterotopia incidence, the incidence of catheter-related phlebitis, catheter-related thrombosis between external jugular vein type I and type II, type II and type III there were no statistically significant difference(P > 0.05), but the differences between external jugular vein type I and type III there were statistically significant difference(P<0.05); The differences of the incidence of secondary catheter heterotopithe and the incidence of ectopic catheter fracture incidence there were no statistically significant difference(P > 0.05).Conclusion:1. Through the research and analysis catheter complications of different external jugular vein types, found that the complications rates of external jugular vein type III higher than type I and type II, so the preliminary draw line that type I and type II is suitable catheter indwelling,the type III should try to avoid.2. Through analysis the different complications of external jugular vein types, the rate of primary catheter heterotopia,catheter-related phlebitis catheter-related thrombosis of type III is higher than type II,but the rates between type I and type II, type II and type III there were no differences.Between different types that the incidence of secondary dislocation incidence rate and catheter fracture was not different.3. Through the external jugular vein classification research and analysis of catheter complications, which can easily be seen in the clear different parting type. The prone to complications of type I were primary catheter heterotopia and secondary catheter heterotopia.The prone to complications of type II were primary catheter heterotopia, catheter-related phlebitis.The prone to complications of type III were primary catheter heterotopia, secondary catheter heterotopia, catheter-related phlebitis, catheter-related thrombosis and catheter fracture. The result provides guidance for clinical nursing of cervical screening indications outside vein classification and implement corresponding catheter nursing measures.
