Background:Peptic ulcer is aglobal common diseases,multiple factor disease,is on e of the main cause of hp infection and non-seroidal anti-inflammatory d rugs,other factors include smoking,genetics and stress,etc. the abdominal pain is the main symptom of this disease.But there are also part of the sy mptoms of patients with asymptomactic or lighter,with bleeding and perf oration as the first symptom.Peptic ulcer is a chronic process,often season al,tend to occur in autumn and winter or during winter and spring alternately. Abdominal pain is often rhythmicity.Gastronintestinal bleeding is the most common complication,can also occur perforation and pyloric obstruction,even a small number of gastric cancer.so the early det ection of gastrointestinal ulcers,for peptic ulcer disease eradication therapy is to cure the digestive tract ulcer and prerent its recurrence,the prevention and treatment of complications of great significance.At present according to the different causes ulcer treetment scheme,no matter whether the existence of H.pylori infection, should be taking Proton pump inhibitions and inhibition of gastric acid scretion. For helicobacter bacilli infection patients, first of all should eradicate H.pylori. But at present our country clarithromycin and metronidazole.levofloxacin resistance increased year by year. Application of standard triple therapy the eradication of H.pylori eradication rate lower them eighty percent,now our country is recommonded to use quadrape therapy containing bismuth agent eradicate H.pylori. In the treatment of peptic ulcer proposed whether peptic ulcer for activity should eradicate H.pylori. But peptic ulcer in the different stages of endoscopic of H.pylori eradication rate is still a lot of research.Foreign scholars think that H.pylori eradication may he associated with gastric mucosa inflammation of different level, remain to be further research.Research purposes:The aims of this study were to evaluate the eradication rates of H. pylori infection according to the endoscopic stage of PUD and to found the regulation and guiding further treatment.Materials and methods:A total of 127 patients with PUD(44gastric ulcer, 83 duodenal ulcer) were retrospecti vely included from July 2015 to December 2015 at the first hospital of Jilin universit. For 77 male patients and 50 female patients, the mean age of the patients was 46.26 ± 12.82 years. The endoscopic stage was classified as active, healing, or scarring.We define the active stage as active ulcer and the healing and scarring stages as inactive ulcer.Active, healing, or scarring periods is 73,33 and 23 cases,respectively. All patients received standard quadruple therapy(PPI, bismuth potassium citrate 110 mg, tinidazole 500 mg and clarithromycin 250 mg twice a day for 14 days).Anyone with a stomachulcer treatment for 8 weeks and duodenal ulcer treatment discontinuatio n 6 weeks after 2-4 weeks review of 14 carbon urea exhalation experimen t and observation of digestive endoscopy under different stages of h.pylor i eradication rate of peptic ulcer.Results:The eradication rates according to endoscopic stage were significantly different in gastric ulcer(active vs healing vs scarring; 80.0% vs 44.4% vs 40.0%, P=0.01, respectively), but there were no significant differences in duodenal ulcer(active vs healing vs scarring; 72.9% vs 70.8%vs 36.4%, P=0.215, respectively). In multivariate analyses, active ulcer was a significantly independent predictor of successful eradication(Odds ratio; 2.660, 95% CI; 1.223–5.784, p =0.014).Conclusion:There was a significant difference in the eradication rate according to the endoscopic stage of gastric ulcer.Active ulcer was an independent predictor of successful eradication.
