Clinical Observation Of Continous Blood Purification On Severe Acute Pancreatitis

Objective:By observing the effect of continous blood purification on severe acute pancreatitis,to explore its clinical value.Methods:Select the patients of Shandong Provical Hospital of Eastern Critical Care Unit from January 2012 to December 2014, that were treated for severe acute pancreatitis,by using continuous blood purification,a total of 34 cases for the study. Compare clinical indicators before trearment and 24h after treatment,including general vital signs、abdominal signs、blood tests index、Acute Physiology and Chromical Health Evalution,etc.Results:34 cases of severe acute pancreatitis,26 cases (76.47%) survived,8 cases (23.53%) died ultimately,1 patient died of acute renal failure,2 patients died of septic shock,3 cases died because their families had gave up treatment.1. Severe acute pancreatitis patients abdominal pain、bloating symptoms improved significantly after continuous blood purification.2.The patient’s basic vital signs, including heart rate (HR), temperature (T), respiratory rate (R), mean arterial pressure (MAP), etc.,decreased significantly after treatment than before treatment, P<0.05, with a significant difference.3.The patient’s blood gas analysis,the oxygenation index (PaO2/FiO2), PH, partial pressure of oxygen (PaO2),increased significantly after treatment than before treatment,P<0.05, with significant difference.4.The blood purification test results in patients,serum amylase (AMS),blood lipase (LPS), blood urea nitrogen (BUN), serum creatinine (SCr), etc.,decreased significantly after treatment than before treatment,P<0.05, with statistically significant difference;procalcitonin(PCT),C-reactive protein (CRP), white blood cell count (WBC) decreased significantly after treatment than before treatment,P<0.05, with a significant difference.5.The acute physiology and chronic health evaluation (APACHE Ⅱ rates),decreased significantly after treatment than before treatment,P<0.05, with significant difference.6.Comparasion of the start time and duration of CRRT treatment between death group and survival group,were significantly different, P<0.01, significant difference.Conclusion:Continuous blood purification is an effective means of treatment of severe acute pancreatitis,which can effectively relieve symptoms,improve biochemical indicators,protect organ function,it has important Promotion and application value.
