Objective:To observe and analyze the clinical efficacy of Continuous veno-venous hemofiltration in severe acute pancreatitis.Methods:73cases of severe acute pancreatitis patients:To divide SAP patients into3groups(A, B, C) on the basis of different causes such as biliary tract diseases, alcohol and others. All of the patients treated with CVVHDF on the basis of comprehensive medical treatment. On the time points about0,24,72h during the treatment and24h after CVVHDF treatment stopped, detect of three groups’ tumor necrosis factor (TNF), interleukin-1,6(IL-1, IL-6), serum amylase, urine amylase, white blood cell, serum creatinine, serum bilirubin Su and blood urea nitrogen. Results:Treatment of73patients,64cases (87.7%) were cured,5patients died (6.85%), because of multiple organ failure,4patients (5.48%) were discharged for other reasons. three groups’ TNF, IL-6and IL-1levels were declined after CVVHDF treatment for24h and72h, while the A group’ index declined slower than group B and C (P<0.05); however, when24h after CVVHDF treatment stopped, A group’ index increased largely than B and C groups’ index (P<0.05), and B and C group’ index chang a little (P>0.05).Conclusions:CVVHDF have a certain effect on varieties of severe acute pancreatitis; it has a great of efficacy of non-gallstone pancreatitis, and early application is better; the treatment which is used on gallstone pancreatitis should last a relatively long time, especially according to the disease, timely using interventions such as surgery or endoscopic to removal of the cause, in order to obtain better results of treatment.
