Current Investigation Of Psychological Stress And Treatment Satisfactory Level Of Chongqing Infertile Female Patients

Objective:To investigate the psychology stress and treatment satisfactory level of Chongqing infertile female patients with the objective to provide basis information for hospitals and related institutions in regards to their treatment and services of their infertile female patients.Method:By using multi-stage randomized sampling method, female patients from infertility clinic within one hospital assisted reproductive technology center from Sep.2012 to Oct.2014 in Chongqing city were participated in this questionnaire. The contents of questionnaire were inclusive of demographic information, duration of infertility, different treatment methods, the number of visits, psychological stress level, infertility knowledge awareness, satisfactory level towards medical healthcare personnel, satisfactory level towards the standard of medical technology etc. A total of 2000 copies of questionnaire were issued in which 1921 copies were retrieved with 1760 copies defined as valid response. The validity rate was 91.62%. Upon the completion of the questionnaires, double personnel double machine double data entry system was applied using EPIDATA software. Using the general descriptive statistical analysis, all the data were entered into SPSS for data analysis. Statistical significance was set at 0.05.Results:Out of the 1760 patients with valid response in this questionnaire,58.30% were timed intercourse patients,24.32% were artificial insemination patients and 17.38% were IVF treatment patients. For the distribution of degree,65.68% was at the age of less than 35 years old,26.48% was between 35-40 years old and 7.84% was more than 40 years old in which the smaller of the age range, the higher ratio it was for the number of patients with university degrees (X2=49.73, P=0.0001).Out of the investigated patients, 84.43% of patients were being categorized as having psychological stress. No statistically significant difference in the number of patients with psychological stress was noted in different age groups. However in different academic level, the number of patients with psychological stress was significantly higher in patients with higher academic level than in patients with lower academic level (X2=8.15, P=0.004) and in different occupational group, the number of patients with psychological stress was also significantly higher in white-collar group than in the blue-collar group or other groups (X2=23.4, P=0.0001). In terms of the duration of infertility, the number of patients with psychological stress was significantly higher in patients with infertility duration period of 2-5 years than in patients with other infertility duration period (X2=24.8, P=0.0001).For different treatment method, the number of patients with psychological stress was significantly higher in IVF treatment group than in timed intercourse group and artificial insemination group (X2=11.7, P=0.003). Patients that had two or beyond two treatment visits were significantly more stressful than those patients with first treatment visit (X2=7, P=0.008). Out of 1486 patients with psychological stress, the stress was mainly from family and society, which was 38.69% and 29.81% respectively.In terms of infertility knowledge awareness,51.70% of patients had shown to have the awareness in which the number of patients with psychological stress was significantly higher in this group of patients than in patient group that had less infertility knowledge awareness (X2=4.1, P=0.028). There was no statistically difference in the number of patients with psychological stress in patient group that understood the infertility treatment process and educational information provided by medical healthcare personnel than in patient group who failed to understand. The number of patients with psychological stress was significantly higher in patient group that understood and accepted artificial insemination as the treatment mode than in patient group that failed to understand (X2=27.15, P=0.0001; X2=4.83, P=0.0028), and in patient group that understood and accepted IVF as the treatment mode than in patient group that failed to understand (X2=6.6, P=0.01; X2=11.03, P=0.0009).87.27% of patients had high expectation of pregnancy after treatment. There was no statistically difference in the number of patients with psychological stress in patient group with high expectation of pregnancy than in patient group with low expectation of pregnancy (X2=3.48, P=0.06).In terms of satisfactory level towards hospital service,82.84% of patients were satisfied with the service of front desk reception staffs in which the number of patients with psychological stress in this group was less than in patient group who was not satisfied with the service of front desk reception staffs (X2=8.8, P=0.003).78.35% of patients were satisfied with doctor’s service attitude in which the number of patients with psychological stress was more than non-satisfied patients (X2=7.09, P=0.008).24.43% of patients were satisfied with doctor’s care towards patients in which the number of patients with psychological stress was less than non-satisfied patients (X2=11.39, P=0.0007).47.78% of patients were satisfied with infertility consultation service provided by the hospital in which the number of patients with psychological stress was less than non-satisfied patients (X2=12.7, P=0.0004). No statistically significant difference in the number of patients with psychological stress was noted in patient groups that were satisfied with nurse’s service and attitude, doctor’s explanation on medical condition and the attitude of personnel on follow-up cases than in patient groups who were not satisfied with those categories.For timed intercourse patients,45.03% of patients were satisfied with the treatment period in which the number of patients with psychological stress was lower than non-satisfied patients (X2=6.97, P=0.008). No significant difference was noted in the number of patients with psychological stress in satisfied patients and non-satisfied patients with doctor’s skill, list of examination topics and chances of pregnancy.For artificial insemination patients,89.48% of patients were satisfied with pre-operation communication with medical healthcare personnel, in which the number of patients with psychological stress was lower than non-satisfied patients (X2=5.51, P=0.02).78.97% of patients were satisfied with treatment cost in which the number of patients with psychological stress was less than non-satisfied patients (X= 6.23, P=0.01). No significant difference was noted in the number of patients with psychological stress in satisfied patients and non-satisfied patients with doctor’s skill, list of examination topics and chances of pregnancy.For IVF treatment patients, there was no significant difference in the number of patients with psychological stress in patients that were satisfied in every index than in patients that were not satisfied.Conclusion:The psychological stress that is commonly present in Chongqing infertile patients cannot be neglected. There is a need to improve patient’s infertility knowledge as well as patient’s medical service and treatment satisfactory level in the hospital. Hospitals need to create more public awareness forums on patient education and healthcare in order to minimize any misleading information as well as to generate family and social care and concern towards infertile patients. Medical healthcare personnel needs to be proactive in communicating with patients, addressing their doubts, consoling their emotions and strengthening patient’s confidence level towards them. The government can consider implementing certain policy as a form of support to infertile patients.
