Objective To study the differences of the clinical characteristics of lung cancer in youth,middle and elderly patients, providing theoretical basis for the early detection and diagnosis of lung cancer in the younth.Methods A total of 252 patients with lung cancer in Zhongshan Hospital Xiamen University has been selected from January 2007 to December 2014 with the method of retrospective analysis. The histology of the tumor was described according to the World Health Organization classification 5 and the stage was defined by the 7th edition of UICC-TNM staging system. According to the World Health Organization on the age division,patients were classified to young group( age ≤ 44 years old), middle age group(45 to 59 years),and elderly group( ≥60 years of age) The clinical characteristics(gender, smoking status, first symptoms, metastasis, carcinoembryonic antigen, neuron specific enolase, pathology)were demonstrated in each group.Result There were 58 cases in the young group(21 males and 37 females), the median age was 35±3.61 years old(range 20~44 years old). 64 cases in the middle group(48 males and 16 females), the median age was 50±3.67 years old(range 45~69 years old). 130 cases in the elderly group(86 males and 44 females), the median age was 74±3.96 years old(range 60~ 88 years old). 1.Gender composition between each group had statistical significance(P<0.05). Compared with the middle, and elderly groups, the incidence of lung cancer in young age group was higher in women(P<0.05). 2.Smoking rate was higher in elderly group than that in young and middle age group(P<0.05). 3.More than three first symptoms in different groups showed statistically significant(P<0.05).4.The incident number of adenoarcinoma, squamous carcinoma and small cell cancer in different groups had statistical significance(P<0.05), large cell cancer and another types of pathology in these three groups had not statistical significance(P>0.05). 5. The lung cancer staging of different groups, CEA has been shown to be lower in middle and elderly group(P<0.05). NSE in these three groups had not statistical significance(P>0.05). 6.The central type and the peripheral type of lung cancer in different groups showed statistically significant(P<0.05). 7.The lung cancer staging of different groups, phase II, phase III, phase IV had statistically significant(P<0.05).Conclusion The number of lung cancer in youth females was significantly higher than that in males, and the incidence of lung cancer in youth females increased than that in middle and elder groups. Smoking was the major risk factor for lung cancer, elderly group had a large number of smoking, the morbidity of lung cancer in elderly group was higer than that in young group.Compared to elderly group, youth patients had more atypical clinical first symptom s and more specificity. The proportion of patients with more symptoms in youth gro up was higher than elderly group. The most common pathological types of lung cancer in youth group was adenocarcinoma. However the proportion of squamous carcinoma in elderly group was relative higher than that in youer group. Youth patients often had a higher clinical stage,more for phase III, phase IV. When compared to young group,CEA has been shown to be lower in middle and elderly group. Patients has more peripheral lung cancer in younger group than in the other two groups.
