Objective: To investigate clinical characteristics and prognostic factors on lung cancer in young patients,and provide reference for clinical treatment and prognosis assess.Methods: clinical data of 133 hospitalized young patients in Fujian Provincial Hospital from January 1,2000 to December 31,2008 were retrospectively reviewed and followed up.COX model was used to analyse the prognostic factors.Results: The proportion of male patients was 44.4%(59/133),and that of female patients was 55.6% (74/133).The ratio of the male and female was 1:1.25.Patients' ages arranged from 17 to 40 years,an average of 33±5.4 years.41.4% (55/133) of the patients showed cough and expectoration as the first symptom.The misdiagnosis rate was 55.6% (74/133). 68.9%(51/74) of the misdiagnosed patients were misdiagnosed as pulmonary infection .56.4% (75/133) of the pathological type was adenocarcinoma .Poorly differentiated tumors accounted for 43.6% (58/133).The proportion of right cancer was 51.1% (68/133). 76.7%(102/133)of the patients were in advanced stage of tumor.The proportion of false negative rate in bronchoscopy was 32.6%(28/86) .The survival time of the patients was from 3.0 to 67.0 months, an average of 21.5±5.6 months.The median survival time was 12.0 months. COX regression analysis indicated that sex,smoking , tumor differentiation, pathological type,stage of TNM,organ metastasis ,lymphonode metastasis,special treatment and serum albumin level were essential prognostic factors .Stage of TNM and organ metastasis were independent prognostic factors.Conclusions: The proportion of female patients was higher than male patients. Respiratory symptoms were the most common symptoms.Misdiagnosis rate was high .Most patients were in the advanced stage when they were finally diagnosed.The proportion of right lung cancer was higher than the left.The tumor differentiation was poor and the main pathological type was adenocarcinoma. High false negative rate was found in bronchofibroscopy.Patients had short survival time and poor prognosis. Sex,smoking,tumor differentiation, pathological type ,stage of TNM, organ metastasis, lymphonode metastasis, special treatment and serum lbumin level were essential prognostic factors.Stage of TNM and organ metastasis were independent prognostic factors.
