Correlation Research Between Brachial-ankle Pulse Wave Velocity And Coronary Heart Disease

Objective: To study the relationship between coronary heart disease and brachialankle pulse wave velocity.Methods: We studied 119 Patienis hospitalized for elective coronary angiography and ba PWV in the De Partment of Cardiology, 2nd Affiliated Hos Pital of Fujian Medicine University during the period of January 2014 to December 2014. According to the result of the coronary angiography(CAG), the patients were divided into CHD group(n=56)and control group(n=63). The CHD group was divided again into three sub-groups:single branch group, double branches group and severe group. The variation characteristics of ba PWV in each group were analysed, and studied between CAD severity assessed by Gensini score and brachial-ankle pulse wave velocity. With the outcomes of CAG as the gold standard, evaluate the efficacy of ba PWV in the screening of CHD by drawing ROC curve.Results: There were statistically significant differences between the CHD group and the control group of ba PWV(P < 0.05).The difference of ba PWV between single branch group(1558.43 ± 97.26cm/s), double branches group(1698.00 ± 100.49cm/s) and severe group(1892.56±209.30 cm/s) was significant(P<0.05).There was a positive correlation between brachial-ankle pulse wave velocity and Gensini score( r=0.886,P<0.01).The best cut-off point for screening of CHD was 1525cm/s,the sensitivity was83.9%, the specificity was 88.9% and Yuedden ’ s index of 0.728 from the characteristic of ROC curve. Hypertention, diabete, age, HCY were prove to be the risk factors for ba PWV>1525cm/s by means of logistic repression test(OR=3.183、2.953、1.112 、 3.091).Hypertention,diabete,ba PWV,smoking,HCY were prove to be the risk factors of CHD by means of logistic repression test(OR=6.813、5.375、78.247、5.932、4.765).Conclusion: we found a positive correlation between CHD and ba PWV,other risk factors.The change of ba PWV can screens CHD and reflect severity of the coronary stenosis.
