Retrospective Analysis Of Pathological Type Of Lung Cancer Inpatients At The Affiliated Hospital Of Yan’an University Over The Period Of 2011~2014

Objectives:The pathological type of lung cancer inpatients at the Affiliated Hospital of Yan’an University over the period of 2011~2014 was analyzed retrospectively, and the dynamic change, sex/age difference and distribution of such pathological type was explored, so as to lay a foundation for the assistance in early diagnosis/treatment of lung cancer and the subsequent epidemiological survey in the Yan’an area. Methods:The pathological type of 496 inpatients with a registered permanent residence in the Yan’an area and definitely diagnosed as primary bronchogenic carcinoma through the pathological examinations at the Affiliated Hospital of Yan’an University over the period of January 2011 ~ December 2014 was analyzed retrospectively. These subjects came from different departments: Respiratory Dept(258 cases), Thoracic Surgery Dept(140 cases) and Oncology Dept(98 cases). The sex, age, residence and pathological type of these subjects were surveyed, and the pathological type and characteristics of lung cancer in the Yan’an area were compared.Results:1. Compared with that for total number of inpatients at our hospital over this period, the change trend for the number of male lung cancer patients was basically same, but that for female ones was not obviously different.2. At our hospital over this period, the proportion of pathological type of lung cancer inpatients did not change significantly.3. The mean onset age of lung cancer of different pathological types.The mean onset age for small cell cancer, adenocarcinoma and squamous carcinoma was 58.83, 60.08 and 61.68 years old respectively. The peak onset age for each pathological type was often 60~69 years old(34.7%). At each age section, the lung cancer was mainly of adenocarcinoma type; and in the young people, the lung cancer was mainly a small cell cancer(36.4%).4. Among 496 lung cancer patients, there were 354 male cases and 142 female cases, i.e. at an overall male/female ratio of 2.49:1.00, including: 78 male cases and 24 female cases in 2011(3.25:1.00); 79 male cases and 37 female cases in 2012(2.14:1.00); 109 male cases and 43 female cases in 2013(2.53:1.00); and 86 male cases and 40 female cases in 2014(1.84:1.00). Therefore, over this period, the male/female ratio tended to decrease year by year, but the proportion of pathological type of lung cancer in male/female inpatients did not change significantly(χ2=2.452, P>0.05).The male/female ratio for squamous carcinoma, small cell cancer and adenocarcinoma was 9.53:1.00; 4.10:1.00 and 1.00:1.11 respectively. The male lung cancer patients were mainly of squamous carcinoma(46.0%), but the female lung cancer patients were mainly of adenocarcinoma(70.1%), i.e. at a difference(χ2=87.492, P<0.05).5. 288 patients came from small towns(58.1%) and 208 patients came from urban area(41.9%). In both small towns and urban areas, the lung cancer patients were mostly of adenocarcinoma(38.54% and 39.90% respectively), but the pathological type was not significantly different(χ2=3.787, P>0.05).6. The lung cancer patients mostly came from Baota District(35.28%), but no lung cancer patients came from Huanglong County. Conclusion:1. At our hospital over this period, the lung cancer inpatients were of adenocarcinoma,(39.1%), squamous carcinoma(36.1%) and small cell cancer(20.6%) at a descending order.2. The lung cancer often occurred at the age of 60~69 years old(34.7%).3. There were more male lung cancer patients. The male lung cancer patients were mainly of squamous carcinoma(46.0%), but the female lung cancer patients were mainly of adenocarcinoma(70.1%). As a whole, the lung cancer was mostly of adenocarcinoma(39.1%).4. In the small towns and urban areas, the pathological type of lung cancer was not significantly different.
