Correlation Analysis Of MRI Manifestations Of Brain Metastases And The Pathological Type Of Primary Lung Cancer

Objective: To explore the relationship between imaging of metastases and the histological type of the primary tumor on the basis of analysis on the MRI characteristics of brain metastases stemming from lung cancer, which provide a reference for clinical treatments, efficacy and prognosis evaluation.Methods: Imaging and clinical data of 137 cases confirmed by pathology or clinical and imaging follow-up of patients with brain metastases stemming from lung cancer were involved in the study retrospectively. Lung cancer is divided into three groups according to the pathological type, 22 cases of squamous carcinoma, 75 cases of adenocarcinoma and 40 cases of small cell lung cancer. All the patients were produced by conventional head MR scan and enhanced scan. The number, size, location, tumor internal hemorrhage and cystic situation, diffuse characteristics of DWI, substance signal of metastases(r ADC), the contrast enhancement and edema range of brain metastases were investigated and recorded. Observations were analyzed using SPSS17.0 statistical software. Chi-square test and rank-sum test was applied to the analysis according to the characteristics of different data. P < 0.05 was used as the standard statistically significant.Results: Chi-square test showed that(1) the number of lung cancer with brain metastasis was statistically significant difference in adenocarcinoma, squamous carcinoma, small cell carcinoma(X~2=6.272, P=0.043). Comparative results between groups showed that the solitary and multiple brain metastasis of lung squamous carcinoma and lung adenocarcinoma was statistically significant difference(P=0.029), so was lung squamous carcinoma and small cell carcinoma(P=0.02).(2)The site of brain metastasis of three groups had no statistically significance(X~2=13.815, P=0.182), the location of brain metastasis had no correlation with histology types of lung cancer.(3) Cystic change of brain metastasis from different histological types was statistically significant difference(X~2=8.02, P=0.018), the probability of cystic change in lung squamous carcinoma was higher than the other groups, while internal hemorrhage of brain metastasis had no statistically significance(X~2=2.633, P=0.268).(4) Diffuse characteristics of DWI of brain metastasis from different histological types was statistically significant difference(X~2=25.063, P<0.001). Comparative results between groups showed that the difference was statistically significance in small cell carcinoma and adenocarcinoma(P<0.001), so was small cell carcinoma and squamous carcinoma(P=0.046). Compared with adenocarcinoma and squamous carcinoma, more brain metastasis of small cell carcinoma showed restricted diffusion.(5) The performance of contrast enhancement of brain metastasis between three groups had no statistically significance(X~2=2.762, P=0.598), the contrast enhancement of brain metastasis had no correlation with histology types of lung cancer. Rank-sum test showed that(1) the size of brain metastases from different histological types was statistically significant difference(X~2=10.02, P=0.007). Comparative results between groups showed that adenocarcinoma and squamous carcinoma(P=0.004), small cell carcinoma and squamous carcinoma(P=0.047)were statistically significance.(2) rADC of metastases was the highest in adenocarcinoma(1.42±0.33), while the situation was reverse in small cell carcinoma(0.83±0.25), the result showed the difference was statistically significant(X~2=63.347, P<0.001). Comparative results showed there were statistically significance between any two groups.(3) Edema range of brain metastases between three groups had no statistically significance(X~2=5.839,P=0.054), the edema range of brain metastasis had no correlation with histology types of lung cancer.Conclusions: The results showed that the size, number, cystic change, diffuse characteristics of DWI and rADC of brain metastases had correlation with pathological type of lung cancer, while the location, tumor internal hemorrhage, the contrast enhancement and edema range of brain metastases had no correlation with pathological type of lung cancer.
