Objective:To explore a simple and effective method of quadriceps muscle exercise through the guidance by arthroscopic operation therapy in patients with knee meniscus injury of quadriceps muscle function training.Methods:30 patients(30 knees) underwent arthroscopic treatment of parallel operation for knee meniscus injury from 2014 August to 2015 February in the Department of orthopedics of Shanxi Dayi Hospital were prospectively analyzed. All of the operations were performed by the same surgery group of doctors, knee the disease knee at 90° position, take a knee before inside, outside entrance, and choose the best operation mode(partial meniscectomy, subtotal resected completely, etc.) according to the meniscus injury type and degree of decision showed by the arthroscopic. After arthroscopic operation, according to the different quadriceps exercise methods, which were randomly divided into A, B, C three groups, respectively, with straight leg raising exercises, static squat exercise, and resistance of knee extension exercise three kinds of training methods, each group consist of 10 patients(10 knees), the groups respectively according to their respective rehabilitation programs for 3 months. Group A consists of 4 male cases and 6 female cases, with an average age of 29.30±9.59 year(range 17-48 years); group B includes 7 male cases and 3 female cases with an average age of 29.70±8.83 year(range 19-45 years); group C was consist of 6 malecases and 4 female cases, with an average age of 29.70±7.33 year(range 22-43 year). There is no significant difference in sex and age among the three groups. Respectively 1 day before operation and 8 weeks after operation, 12 weeks after operation, measure the thigh circumference of knee superior border of patella 10 cm above, then calculate the thigh circumference atrophy index [thigh circumference atrophy index =(contralateral thigh circumference- ipsilateral thigh circumference) / contralateral thigh circumference] and record Lysholm of knee joint assessment scale. Statistical analysis was conducted with the SPSS software(SPSS13.0 Chicago, III). Difference in the groups were analyzed with matched t-test, analysis of variance(ANOVA) was for comparison between preoperative and latest follow-up. The significance level was set at P<0.05.Results:The three groups’ superior border of patella 10 cm thigh circumference atrophy indexes of 1 day before operation were 0.026?0.015, 0.021?0.016, 0.037?0.021, no significant difference(P>0.05); 8 weeks after operation of superior border of patella 10 cm where the thigh circumference atrophy indexes were 0.021?0.013, 0.016?0.017, 0.023?0.016, no significant difference(P>0.05); 12 weeks after operation of superior border of patella 10 cm where the thigh circumference atrophy indexes were 0.015?0.008, 0.011?0.013, 0.014?0.011, no significant difference(P>0.05). The thigh circumference atrophy index has no obvious statistical difference among the three groups when it came between 8 weeks after the operation and 12 weeks after operation. But it was different in group C. The thigh circumference atrophy index comparison of group C had significant difference 1 day before operation and 12 weeks after operation. However, the result is negative between 1 day before operation and 8 weeks after operation and between 8 weeks after operation and 12 weeks afteroperation. Group A and group B in the comparison among groups had no statistically significant differences. There were no statistically significant differences of the three groups’ Lysholm scores among group A, B and C.Conclusion:There were no significant statistical difference among the three training methods on quadriceps muscle atrophy and muscle strength rehabitation in the study. But it was different in the resistance stretching exercise group. The study found that resistance stretching exercise for quadriceps could promote the recovery of quadriceps atrophy and the muscle strength weak and improve patients with Lysholm score.
