Application Of Sequential Therapy About Dental Implant In The Aesthetic Restoration Of Traumatic Dentition Defect

Object:To observe the clinical effects of dental implants in the aesthetic restoration of traumatic dentition defect patients caused by serious trauma.Methods:A total of 18 patients with bone defect in implantation area were selected as subject,who were included 7 patients with more than 2 tooth and alveolar bone damage.6 patients with occlusal disorders.1、Preparing for implant surgery and design the treatment planing of the sequential therapy:(1)General take comprehensive physical examination and X-ray examination before implant surgery to understand the situation of alveolar bone in the implantation area.Then design the treatment planning of the sequential therapy.(2)After exodontia,some technology were used to keep the bone and soft tissue conditioning.(3)Patients with dentition malformation were treated with orthodontic techniques before or after implant restoration.2、Surgery procedure:(1)According the preoperative design to performimplant surgery.(2)Bone defects around implants were repaired by guided bone regeneration technique with bone graft,put the appropriate size membrane on the bone defect area,and over the edge of bone graft 2-3mm.(3)Close the incision and prophylactic use antibiotics.Remove the stitch after 7days.3、Temporary restorations were used in the transition period :Temporary restorations were used to make the tissue conditioning of implants.Adhesive partial denture was the most popular in our research.4、After second stage surgery implant supported temporary restoration was used to make the tissue conditioning of implants:3-6 months later second stage surgery was performed and at the same time we transplant soft tissue.Implant supported temporary restoration was used to make the tissue conditioning of implants5、Restorations were made:then using custom-made transfer coping to give technician precise soft tissue information,restorations were made.6、We use pink aesthetic score to detect the aesthetic effect.Result:In this group of 18 patients,we implant 32 implants during the first surgery.8 cases in group received immediate implant and 6 cases receiveddelayed implant.During the 1-2 years of clinical observation after the restoration,we find that all the implants are in good stability,the upper side functions well and the soft tissue is in good shape.That patients are all satisfied with the results and the success rate is 88.9%.The index of the mean PES was 6.59±0.67 that higher then(4.96 ±0.89)which was the mean PES of previous phase.There were significant differences(P<0.05).Conclusion:The clinical effects of dental implants in the aesthetic restoration of traumatic dentition defect patients caused by serious trauma was good method.
