Serum Fibroblast Growth Factor 21 Level Is Associated With Left Ventricular Hypertrophy And Lipid Metabolic Disorder In Nondiabetic Patients Receiving Continuous Ambulatory Peritoneal Dialysis

Abstract/Summary:
Objective:The purpose of this study is to analyze the factors which influence serum FGF21 level in nondiabetic patients receiving continuous ambulatory peritoneal dialysis; to explore the correlation between serum FGF21 level and left ventricular hypertrophy、blood lipids and micro inflammatory state in nondiabetic patients receiving continuous ambulatory peritoneal dialysis, and further to investigate if serum FGF21 level can be as one of the most important cardiovascular events’ s predictors in non-diabetes CAPD patients.Method:①82 patients with non-diabetes CAPD were enrolled as research subjects in peritoneal dialysis center of General Hospital of Ningxia Medical University from December2013 to November.2014; At the same time,40 cases of normal people who take the health examination in our hospital medical center as control subjects;②Collecting patients’ general clinical data and correlated laboratory examinations using ELISA to detect serum FGF21level;③Serum FGF21 levels were compared between the two groups, Further being divided into different sub groups based on whether there are left ventricular hypertrophy 、hyperlipidemia、elevated hs-CRP and hypertension, To investigate the correlation between serum FGF21 level and non diabetic CAPD patients with left ventricular hypertrophy,dyslipidemia, micro inflammation state. Univariable analyses and multiple stepwise regression analyses were used to analyze the associations between FGF21 and relative factors;Results:① A total of 82 participants were nondiabetic CAPD patients(43men and 39women) with a mean age of 47.17±11.39 years. 40 participants were normal control group(23 men and 17 women) with a mean age of 43.43±9.80 years. The age and gender of nondiabetic CAPD group and normal control group have no statistically significant difference(P>0.05);②Compared with controls, serum FGF21 concentration was 1.72 times higher in nondiabetic patients with CAPD and LVMI was higher than controls, The difference was statistically significant(P<0.001), The incidence of left ventricular hypertrophy can be as high as 59.8%;③serum FGF21 levels in nondiabetic CAPD patients with LVH were higher than those in patients without LVH(P=0.013) and normal controls(P<0.001), moreover, serum FGF21 levels in nondiabetic CAPD patients without LVH were higher than normal controls(P=0.045);④In non diabetic patients with CAPD,serum FGF21 level of dyslipidemia group was significantly higher than the normal blood lipid group(P=0.031);⑤multiple stepwise regression model analysis showed FGF21 in non diabetic patients with CAPD was positively correlated with LVMI(β=0.305,P=0.006).Conclusion:The serum FGF21 level is closely associated with cardiovascular disease, serum FGF21 level in nondiabetic patients with CAPD increased significantly, increased higher with left ventricular hypertrophy and dyslipidemia. Serum FGF21 level in nondiabetic CAPD patients correlated positively with LVMI. And left ventricular hypertrophy and lipid metabolism disorder are the most common cardiovascular disease complications. therefore,serum FGF21 level may become one of the important predictors as nondiabetic CAPD patients with cardiovascular events.
