Development And Clinical Application Study Of The Portable Electrical Stimulation Therapy Apparatus For Brachial Plexus Injuries

Objective:According to the characteristics of the brachial plexus injury of nerve electrical stimulation treatment,and the successful development of a single-channel electrical nerve stimulation therapy apparatus for the single peripheral nerve injury on the basis of clinical validation, the author of this paper has developed a kind of portable three-channel brachia plexus injury electrical stimulation apparatus exclusively for curing the brachia plexus injury, applied them to the patients with brachia plexus injury,and then observe the clinical therapeutic effect.Methods:(1)Based on the principles of the functional electrical stimulation of electrophysiology research,the author of this paper developed of a kind of single channel nerve electrical stimulation therapy apparatus.The system hardware parts composed of the microcontroller, power module, display module, parameter storage module, electrode shedding detection circuit, battery voltage detection circuit,high voltage pulse generating circuit and so on. The system software is based on the ARM kernel hardware platform through software programming.Through treatment of software process control procedures,the therapy apparatus has realized the some kinds of different stimulation waveform. Via the self-adhesive electrodes on the surface,it can finish the output of pulse wave, and apply it to the different types of single peripheral nerve.(2) Randomly divide 65 postoperative patients(20 cases of ulnar nerve,20 cases of radial nerve, 25 cases of median nerve) with single peripheral nerve injury into an experimental group(35 cases) and a contrast group(30 cases). The experimental group whose stitches were taken out in the second week after the operation were applied with the developed portable single-channel electrical nerve stimulation therapy apparatus to treat them, and took neurotrophy medicine on time and do active and passive function exercise on the diseased limb; the contrast group took the neurotrophy medicine on time and do active and passive function exercise on the diseased limb. They were followed up as schedule after operation and observed their injured neural functional recovery status after6 months.(3) For the single channel nerve stimulation therapy apparatus can only cure a single nerve, and according to the characteristics of the brachial plexus for more groups of nerve injury, the author of this paper developed a kind of three-channel nerve stimulation therapy apparatus.The portable three-channel electrical stimulation system composed of the master control module, display module, keyboard module, and three-channel electrical stimulation generator module and power supply module releases three groups of continuous alternative electric pulse stimulation, and performs alternative output according to Channel 1, 2, and 3 continuously. Besides, the stimulation of each channel lasts for 1-5 minutes.(4) Randomly divide 28 postoperative patients with brachial plexus injury in C5-C7 into an experimental group(15 cases) and a contrast group(13 cases). In the third week after the operation, the experimental group adopted the developed portable three-channel brachia plexus injury electrical stimulation apparatus exclusively for curing the brachia plexus injury. The contrast group did not do any treatment. The patients were followedafter the operation. After eight months, observe the patients’ upper limb, shoulder, and elbow function recovery status.Results:(1) The portable single-channel electrical nerve stimulation therapy apparatus is applied to treat the postoperative patients with the single peripheral nerve injury. It turns out that the good rate is obviously better than that of the contrast group, and the difference has statistical significance(P<0.05).( 2) After the portable three-channel brachia plexus injury electrical stimulation apparatus is used to cure the postoperative patients with the brachial plexus injury in C5-C7, their functions of shoulders and elbows are better than those before the treatment.The difference is obvious and has statistical significance(P<0.05).Conclusions:(1)The portable single-channel electrical nerve stimulation therapy apparatus can effectively accelerate the function recovery after single nerve injury. Besides, the therapy mode can be personally adjusted, with low cost, flexible usage, selectivity and security.(2)The successful three-channel brachia plexus injury electrical stimulation apparatus can effectively accelerate the recovery of the upper limb function after the brachial plexus injury. Besides, it can exert continuous alternative electrical nerve stimulation on three groups of nerves and muscles, with relatively high innovation, good effect, convenient usage, and portable feature. The patients are satisfied with the therapy.
