The Clinical Study Of Laparoscopic Total Extraperitoneal Hernia Repair Without The Use Of The Balloon Dissector And Fixing Patch

Objective To compare the effect of laparoscopic total extraperitoneal hernia repair without the use of the balloon dissector and fixing patch and Open tension-free herniorrhaphy. To explore the feasibility of laparoscopic total extraperitoneal hernia repair without the use of the balloon dissector and Fixing patch. Method Choose General Surgery of the First Affiliated Hospital of Baotou Medical College. A total of 46 inguinal hernia patients admitted to hospital from February 2012 to October 2013 were enrolled in the study. The patients made retrospectively analyzed, including 23 cases of open tension-free herniorrhaphy. The open group employed epidural anesthesia, while TEP group of 23 cases of laparoscopic total extraperitoneal hernia repair without the use of the balloon dissector and fixing patch adopted general anesthesia. Observe and take notes of index, such as the operating time, blood loss in the operation, the postoperative hospital stay, operation complications, cases of using analgesic, and the cost of hospitalization. Results The operating time: open group(54.6±10.0)min,TEP group(45.4±5.8)min,(P<0.05),with statistical significance;Blood loss in the operation: open group(37.0±10.6)ml,TEP group(19.6±7.7)ml,(P<0.05),with statistical significance;The postoperative hospital stay:open group(5.3±1.7)d,TEP group(2.4±0.5)d,(P<0.05),with statistical significance;The cost of hospitalization: open group(9952.7±2180.6)yuan,TEP group(9952.7±2180.6)yuan,(P>0.05),No statistical significance;Cases of using analgesic: open group 14 cases,TEP group 2 cases;Operation complications :open group :6 cases in total, including 3 cases of scrotum seroma,2 cases of wound seroma, and 1 testis twist,TEP group :2 cases in total, all of which are scrotum seroma.By compare,( P>0.05),there is no statistical significance on operation complications and the difference of rate in the total complications. No recurrence in operation area was found during the follow-up of 2-24 months. Conclusions TEP operation without the use of the balloon dissector and fixing patch is clearly superior to open tension-free herniorrhaphy on operating time, blood loss in the operation, postoperative hospital stay and so on. There is no obvious difference between two operations on operation complications and the cost of hospitalization, however. Thus it can be seen TEP operation without the use of the balloon dissector and fixing patch and traditional herniorrhaphy can cure inguinal hernia completely, but TEP operation has more advantages than traditional herniorrhaphy. TEP operation without the use of the balloon dissector and fixing patch is a minimally invasive operation which is safe and accessible. Also, it is possible to be applied in hospitals at local level.
