Objectives With aggravated aging, socio-economic development and the improvement of people’s living standards, chronic disease has become a major public health problem in the elderly and affected the quality life of residents seriously, with high incidence and high prevalence, high morbidity, high mortality and high costs features.In this study, for the active prevention and control of chronic diseases and improve the quality of life of older people, we understand the status and analyze the influential factors of the chronic illness in the elderly people, and providing a scientific basis for chronic disease management programs and healthy aging.Methods During July 2012 to May 2013, Totally 6173 elderly people aged 60 years and above were recruited in three regions of Hebei Province by multi-stage stratified cluster sampling method, they were investigated with questionnaires, including general information survey of elderly people(urban and rural, gender, age, educational level, marital status, income, working condition, et al), Chronic disease self-designed Questionnaire, AHSMSRS, FAD, SSRS and PSQI. All data were established with EXC EL and analyzed with SPSS13.0 package. Specific methods: Univariate analysis of count data were performed using the chi-square test, the variables of multivariate analysis from the univariate analysis with statistical significance: the urban and rural areas, age, gender, marital status, education level, work statue, smoking, exercising, constipation, sleep disorders, balance of payments, the type of health insurance, pension insurance, social support, health self- management, number of children, family urge medical care and negative life events as independent variables, whether suffering from chronic disease or not as the dependent variable, for taking logistic regression analysis.Results 1 The status of chronic illness in the elderly people: ①The species distribution of the elderly people of suffering from chronic diseases: The total number of patients older 4633, accounting for 75.1%, suffering from one kind of chronic disease were 1641 elderly people, accounting for 26.6%, suffering two kinds were 1207 elderly people, accounting for 19.6%, suffering from three kinds were 734 people, accounting for 11.9 %, suffering from four kinds were 371 people, accounting for 6.0%, and more than five kinds were 680 people, accounting for 11.0%. ② The status of the elderly people suffering from chronic diseases in urban and rural: The elderly people’s prevalence and suffering from 1-3 kinds chronic diseases in urban were higher than rural, while suffering from above 4 kinds chronic diseases in rural elderly were higher. The top five diseases of overall prevalence were hypertension, heart disease, rheumatoid arthritis, diabetes, osteoporosis. The top five in urban were hypertension, heart disease, diabetes, rheumatic arthritis, osteoporosis. The top five in rural areas were hypertension, cardiovascular and cerebrovascular diseases, rheumatoid arthritis, heart disease, osteoporosis, The urban elderly’s prevalence of hypertension, heart disease, diabetes were higher than the rural elderly, other diseases prevalence in rural elderly were higher, the differences between urban and rural were statistically significant(P<0.05). ③The elderly people’s prevalence and suffering from above 2 kinds chronic diseases in women were higher than men; ④The elderly above 70 years old were in higher prevalence, but above 85 years old decreased. 2 Single factor analysis of chronic illness in the elderly people: chronic diseases prevalence of the elderly were significant in marital status, education level, work condition, smoking, alcohol consumption, exercise, constipation or not, sleep disorders, balance of payments, health insurance type, pension insurance, social support, health selfmanagement, the number of children, medical treatment from families’ urgent(P<0.05). 3 Multiple factors analysis of chronic illness in the elderly people: Logistic regression results showed that the influential factors of suffering from chronic diseases were: constipation, negative life events, sleep disorders, family urge medical care, health self- management, the number of children, social support, working condition, smoking, there is statistical significance(P<0.05).Conclusions 1 The overall prevalence of the elderly people were 75.1%, and most elderly people were coexisted with 1-3 kinds chronic diseases. The top five prevalence of chronic diseases were hypertension, heart disease, rheumatoid arthritis, diabetes and osteoporosis; The elderly people’s prevalence of chronic diseases in urban, women were higher, the elderly above 70 years old were in higher prevalence, but above 85 years old decreased. 2 Multivariate results showed that the influential factors of suffering from chronic diseases were: constipation, negative life events, sleep disorders, family urge medical care, health self- management, the number of children, social support, working condition, smoking.
