Study On RANKL/OPG In Gingival Crevicular Fluid Of Postmenopausal Patients With Periodontitis

Objectives To collect the gingival crevicular fluid(GCF) of women periodontitis and control groups, and test contents of receptor activator nuclear factor kappa B ligand(RANKL) and osteoprotegerin(OPG). Observing the difference between the groups, it is discussed that what are effects of premenopause periodontitis and postmenopause periodontitis, it will provide theoretical evidence on the prevention and clinical treatment.Methods 1 Comparing two kinds of materials of collecting GCF: Whatman I filter paper for 2×10mm long and 30 th absorbent paper points for 10 mm long are selected to study protein contents and elution protein concentration, which used BCA in vitro. 2 The choice of subjects: Selecting female patients(age varying from 45 to 65 years old)from September 2013 to September 2014 in Department of stomatology, Tangshan people’s hospital, among whom are 30 premenopause healthy subjects, 30 premenopause periodontitis patients, 30 postmenopause healthy subjects and 30 postmenopause periodontitis patients. Accepting criteria: tooth with complete shape, tooth neck without decay, without fillings, without gingivitis, the clinical index of healthy subjects are gingival index≤1, plaque index≤1, attachment loss≤1mm, or probing depth≤3mm, the clinical index of periodontitis patients are gingival index>1, plaque index>1,attachment loss ≥3 mm, or probing depth≥4 mm. Exclusion criteria: history of smoking, using nonsteroidal anti-inflammatory medicals and other drugs history in past 3 months, systemic diseases such as diabetes, hypertension, cardiovascular disease, inflammatory diseases,local infection, fungal or virus infections, rheumatoid arthritis, other types of periodontitis and recently receiving periodontal treatment. 3 We choose moisture absorption paper point method to extract gingival crevicular fluid and put the finished samples immediately into the- 70 ℃ low temperature refrigerator. Choosing ELISA to test contents of RANKL and OPG. 4 Using SPSS software for data analysis.Results 1 Comparing protein contents of different kinds of materials of collecting GCF:30th absorbent paper points for 10 mm long, content 0.126±0.006μg/ml; Whatman I filter paper for 2×10mm size, content 0.123±0.005μg/ml. The comparing results has no obvious difference(P>0.05). 2 Comparing elution protein concentration after absorbing protein of two kinds of materials: the first kind of materials content 3.494±0.300μg/ml; the second kind of materials content 3.233±0.419μg/ml; The comparing results has no obvious difference(P>0.05). 3 The attachment level of the postmenopause group with periodontitis is 4.633±1.129 mm, which was higher than that of premenopause group with periodontitis(4.033±0.964mm)(P<0.05). 4 The levels of OPG in GCF of the periodontitis groups was significantly lower than that of the control groups(P<0.01), but the levels of RANKL and RANKL/OPG in GCF of the periodontitis groups were both significantly higher than that of control groups(P<0.01). 5 The levels of OPG in GCF of the postmenopause group with periodontitis(64.267±7.332 pg/μl) was significantly lower than that of the premenopause group with periodontitis(71.300±7.292 pg/μl)( P<0.01),but The levels of RANKL and RANKL/OPG in GCF of the postmenopause group with periodontitis(128.386±12.654 pg/μl and 2.020±0.267) were both significantly higher than that of premenopause group with periodontitis(119.914±15.921 pg/μland 1.700±0.268)(P<0.01).Conclusions 1 They can be used to collect GCF. But absorbent paper point is prepared easily, which is easy to collect samples of GCF but not easy to hurt gingival. 2 The expression of OPG was decreased, while the expression of RANKL was increased in GCF of periodontitis. It is indicated that the imbanlance of OPG and RANKL may be primary reason resulting in periodontitis. 3 The expression of OPG was decreased, while the expression of RANKL and RANKL/OPG was increased in GCF of premenopause group with periodontitis. Therefore, it is suggested that RANKL/OPG is main cause leading to periodontitis, RANKL/OPG was probably served as an important indicator of clinical detection of postmenopausal women periodontitis.
