Objective: 1.To discuss the predisposing factors and summarize clinical characteristics of acute gouty arthritis patients. 2.To analyze the relation between serum uric acid level and serum lipid of acute gouty arthritis.Methods: 1. Gout patients from the First Affiliated Hospital of Wan-Nan Medical College was selected. Recruitment occurred between October 2013 and September 2014. Questionnaire survey, clinical analysis, and laboratory measures were obtained for each participant. 2. 227 cases male patients of acute gout attack were collected between 2011 and 2014 from the First Affiliated Hospital of Wan-Nan Medical College. Then, their general information and biochemical indexes were recorded. Last, analyze the correlation of the blood lipid and blood uric acid.Results: 1.The peak onset age of all the gouty patients who were selected from the First Affiliated Hospital of Wan-Nan Medical College between October 2013 and September 2014 was 50-60 years old. The proportion of males and females were 20 to 1. 88.03% of the above patients has definite triggers before attacks. High purine food induced gouty arthritis is 52.14%, alcohol is 47.01%. 2. Familial heredity was found in 16.24% of these gouty patients. The typical initial presentation of gout is podagra. At the attacked, the first metatarsophalangeal joint was involved among 51.28%. Gouty patients with young age of first onset, long duration and gout stone existed were susceptible to have high frequency of attacks. Only 55.56% patients had a higher serum uric acid than 420umol/L when attacked. The level of serum uric acid had positive correlation with blood urea nitrogen, serum creatinine and urinary inhibition-C. In above patients, tophus was found in 37(31.62%) patients, and tophus affected hands, feet, and so on. Frequent attacks, biood urea nitrogen, age, BMI, and long duration were risk factors for gout stone formation, according to the multi-factor logistic regression analysis, age,frequency attacks and duration were the independent risk factors for the information of tophi. 3. Markedly different results were observed for selected inpatients of acute gouty arthritis from the First Affiliated Hospital of Wan Nan Medical College between 2011- 2014 and healthy subjects according to the plasma levels of total cholestrol(TC4.11 ± 1.01vs3.01 ± 1.83,P<0.05)and triglyceride(TG1.44 ± 0.87vs1.01 ± 0.66,P<0.05), but no differences were seen in other components of blood lipid. 4. Gouty patients were grouped by dyslipidemia(with pure higher TC for high TC group, with pure elevated TG for high TG group, TG and TC increase as high TG + group TC, TG and TC normal blood lipids group).The mean serum uric acid of high TG and TC group was highest(531.61 ± 144.91), high TG group second(514.27 ± 133.34), high TC group and the normal group lower(475.56 ± 145.32,460.59 ± 140.71). 5. Correlation analysis indicated that the improvement in blood uric acid may be not closely associated with the increasing of TG level in acute episode phase group(r=0.173,P<0.01). No correlation existed between serum uric acid and total cholesterol, LDL-C, HDL-C(P>0.01).Conclusion: 1. Almost patients are middle-aged and older with a significant sexy distribution(20 to 1). High purine diet and alcohol use are the commonest of the inducements of acute gouty arthritis. 2. Gouty patients with young age of first onset, long duration and gout stone existed are susceptible to have high frequency of attacks. The serum uric acid level is not high in large proportion of gouty patients in acute episode phase. Older age, frequency attacks and long duration are susceptible for the information of tophi. 3. Compared with healthy controls, acute gouty patients has an extremely higher level of triglyceride and TC mild increased in gouty patients of acute episode phase. 4. Triglyceride and serum uric acid level is not closely positively correlated.
