| Objective:To explore the clinical characteristics of patients suffered from gout with different ages of onset,and offer a reference for clinical.Methods:The patients who were diagnosed as gout in the Department of Rheumatology,Ning Xia University General Hospital from January 2013 to January 2018 were collected,and divided into three groups according to the age of onset.the young age group:age of onset<45 years old,the middle age group:45 years old≤age of onset<60 years old;he elderly age group:age of onset≥60 years old.The patient’s general condition,inducement,course of disease,initial joint,number of affected joints,tophi,comorbidities,complications,laboratory tests,etc.were all collected,and the results were evaluated using statistical methods.Results:1.General condition:(1)for all the patients,306 were in young age group,114 were in the middle age group,and 35 were in the elderly age group.The highest frequency of onset age is25-45 years old[264 cases(58.02%)];(2)The proportion of male patients in the three groups was significantly higher than female[305(99.7%)in the young age group,106(93.0%)in the middle age group,and 30(85.7%)in the elderly age group,P<0.05];(3)The proportion of female patients with gout in the middle age group and the elderly age group was larger than the young age group[1 case(14.3%)in the young age group,8 cases(7%)in the middle age group,and 5 case(14.3%)in the elderly age group,P<0.05];(4)The BMI in young age group is highest,the second comes the middle age group,and the elderly age group is the lowest[young age group(26.99±3.52 kg/m~2),middle age group(25.99±2.89kg/m~2),and the elderly age group(24.49±3.69 kg/m~2),P<0.05)].2.Inducement:The most common cause for the onset of gout in young age group and middle age group is drinking[135 cases(44.1%)in the young age group,45 cases(39.5%)in the middle age group,and 5 cases(14.3%)in the elderly age group,P<0.05];There was no obvious incentives for the onset of gout in elderly age group[92 cases(30.1%)in the young age group,34 cases(29.8%)in the middle age group,and 18 cases(51.4%)in the elderly age group,P<0.05].3.The joint which was the most common initial joint in the three groups was the first metatarsophalangeal joint[190 cases(62.1%)in the young age group,76 cases(66.7%)in the middle age group,and 25 cases(71.4%)in the elderly age group,P>0.05].4.Comorbidity:(1)The proportion of patients companied with hypertension in the middle-age and the elderly age group were higher than the young age group[85 cases(27.8%)in the young age group,69 cases(60.5%)in the middle age group,and 20 cases(57.1%)in the elderly age group,P<0.05];(2)The proportion of patients companied with with coronary heart disease in the middle age group and elderly age group was higher than the young age group[10 cases(3.3%)in the young age group,13 cases(11.4%)in the middle age group,and5 cases(14.3%)in the elderly age group,P<0.05].5.Complication:The proportion of patients companied with renal damage in the elderly age group was highest,the middle age group comes second,and the third in the young age group[75 cases(24.5%)in the young age group,64 cases(56.1%)in the middle age group,and 27 cases(77.1%)in the elderly age group,P<0.05].6.Laboratory tests:(1)The average level of WBC in the young age group and the middle age group was higher than the elderly age group[young group(7.92±2.66)*10~9/L,middle-aged group(7.46±2.34)*10~9/L,and elderly group(6.47±1.94)*10~9/L,P<0.05];(2)The level of UA in the young age group was higher than the middle age group and the elderly age group[young age group(510.86±123.51)umol/L,middle age group(457±129.53)umol/L,and elderly age group(444.29±112.45),P<0.05];(3)The level of TG in the young age group was higher than the middle age group and the elderly age group[young group 2.16(1.42,2.98)mmol/L,middle age group 1.79(1.30,2.67)mmol/L,and elderly age group 1.47(1.15,2.23)mmol/L).,P<0.05];(4)The level of ALT in the young age group was higher than the middle-aged group and the elderly age group[32.9(21.10,50.40)U/L for the youth group,27.25(17.53,38.48)U/L for the middle-age group,and 25.50(17.75,35.50)U/L for the elderly group.P<0.05].Conclusion:1.The morbidity of gout in the young age people s higher than in the middle-aged and older people;2.The proportion of patients companied with hypertension,coronary heart disease and renal damage in the middle-aged group and the elderly age group were higher than the young age group.3.The level of BMI,WBC,UA,TG,and ALT in patients with gout were higher in the young age group than in the elderly age group. |
