Research purposes:1, this study proposed to add and subtract yam pill in patients with mild cognitive impairment in clinical early intervention, to carry on the simple intelligence scale for mental state examination (MMSE), Montreal cognitive assessment scale (MoCA) testing evaluation, to observe the change of cognitive function in patients before and after the treatment, and then explore the internal mechanism of the party to improve cognitive impairment, for a new treatment approach for the prevention and treatment of mild cognitive dysfunction, further research to the pathogenesis of mild cognitive impairment provide new theoretical exploration.2, at the same time intends to set up experimental drugs to aging rats model, based on the viewpoint of concentration-response relationship, add and subtract yam pill exposition the improvement of cognitive ability of aging rats, to explore the party the effects of oxidative stress of rats had revealed to add and subtract yam pill in the treatment of mild cognitive impairment related pharmacological mechanism, provides the basic research basis for the clinical treatment of the disease.Research methods:1 cases, clinical research from October 2013 to October 2014 in hubei province hospital of geriatrics department, general hospital of guangzhou military area commands wuhan nerve internal medicine outpatient and inpatient, a total of 60 cases, were randomly divided into treatment group and control group. Among them, the treatment group 30 cases,30 cases of control group were given to add and subtract yam pills, capsules olathe treatment, a course of 3 months. Treatment group and control group in before 2 weeks, 2 weeks after treatment respectively related cognitive function test (MMSE test, MoCA testing). Research data with SPSS16.0 statistical software, and comparison in the group using single factor analysis of variance, group of measurement data comparison between the independent sample t-test, count data by chi-square test, P<0.05 for the difference was statistically significant. The blood, urine, dung routine before and after treatment, blood glucose, liver and kidney function, electrocardiogram (ecg) and other routine test.2, SPF 60 adult male Wistar rats, provided by the centers for disease control and prevention of hubei province animal experiment center, weight 200+20 g. The 60 rats were randomly divided into 6 groups:normal group, model group, olathe temple capsule group, add and subtract yam bolus dose in low dose group, add and subtract yam pill group, the add and subtract yam pills high-dose group,10 in each group. Produced by subcutaneous injection of D-galactose experimental state of subacute aging rats, and by Morris water maze to assess level of model rats with mild cognitive impairment,and then give to fill the stomach medicine, source of drugs and its preparation are supplied by the hubei province hospital of TCM pharmacy and pharmacy department and by physical method was condensed per milliliter contained 1.0 g of native medicinal materials. Experimental rats executed by colorimetric method after rat hippocampal tissue homogenate SOD activity and MDA content.The results of the study:1, MMSE score comparison:using MMSE scale test, the results obtained with statistical analysis, two groups total effective rate difference was statistically significant (P<0.05), and subtract yam pill in olathe capsule on mild cognitive impairment are improved, and the curative effect is superior to the latter.2, MoCA score comparison:the MoCA scale test and data analysis after statistics processing, two groups total effective rate difference was statistically significant (P< 0.05), and subtract yam pill in olathe capsule on mild cognitive impairment were improved, and the curative effect is superior to the latter.3, TCM syndrome curative effect comparison:basic,10 cases were healed, the treatment group,12 cases were markedly effective, 6 cases effective, ineffective in 2 cases, total effective rate was 93.33%; Control group basic recovery 6 cases,11 cases were markedly effective, effective in 5 cases,8 no effect, the total effective rate was 73.33%; After statistics analysis, the treatment group and control group total effective rate difference was statistically significant (P< 0.05).4, hemorheology comparison:two groups after treatment indexes were obviously improved than before treatment, after statistics processing analysis of the differences between the two groups was statistically significant (P<0.05), and subtract yam pill in olathe capsule hemorrheology in patients with mild cognitive impairment status has improved, and the curative effect is superior to the latter.Compare 5, medication safety, observe whether the two groups of adverse reactions in treatment and side effects, and the results show that the two sets of observation and no obvious adverse reaction occurred during severe side effects, the two groups before and after treatment of laboratory results no abnormal changes occurred.6, the Morris water maze test to evaluate model rats behavior change situation, compared with normal group, model group rats positioning navigation and space exploration ability were significantly decreased (P< 0.01), prove the experiment preparation of cognitive dysfunction in the aging rat the affirmation.7, compared with normal group, model group group rats hippocampus increased SOD activity and MDA content, confirmed by subcutaneous injection of D-galactose prepared subacute aging rat model is feasible and safe coefficient is high, not seen obvious abnormity of rats after building.8,6 groups of oxidative stress reaction index (SOD activity and MDA content), statistically significant difference between model group and normal group (P< 0.01); Add and subtract yam bolus dose group index contrast olathe capsule group has increased significantly, the difference is statistically significant (P< o. 05); Add and subtract yam pill low, high dose group olathe efficacy capsule group, the rising trend is not obvious, there was no statistically significant difference (P>0.05).Research conclusions:1, the clinical observation results show that add and subtract yam pill each treatment group in olathe capsule in MMSE score, MoCA score, the comparison of TCM syndrome and blood rheology change, etc., after statistics analysis, had no significant difference, that add and subtract the clinical curative effect of Chinese yam pill in the treatment of mild cognitive impairment, it can significantly improve the clinical symptoms in patients with mild cognitive impairment, improve the quality of life, improve the MMSE score, MoCA score, improve the patient’s blood rheology, has good clinical application prospects.2, add and subtract yam pill can obviously improve the aging rat tissue SOD activity and reduce MDA content, adjust the metabolism of oxygen free radicals, improve hemodynamics of rats. Add and subtract yam bolus dose group is obviously better than the effect of the low and high dose group, at the same time add and subtract yam bolus dose group curative effect is superior to western medicine olathe capsule in the control group, prove concentration-response relationship for the important influence of mild cognitive impairment, drug intervention in provide experimental support for clinical high-dose medication at the same time, also the basis of experimental study on the safety of medication provides a new way of thinking.
