Background:Removable partial denture is one of the main forms of repairing partially edentulous arches. There are many types of RPD. According to the support, there are three types of RPD,which include tooth borne RPD, tooth-tissue-borne RPD and mucosa borne RPD. The way to repairing the distal denture are often tooth-tissue-borne RPD. However, tooth-tissue-borne RPD is more complex repair methods in clinical. Due to the difference in support, this difference will cause the tissueward motion of distal-extension when the denture under occlusal pressure. Meanwhile, it not only can create a larger lateral torsional rotational to the distal abutment teeth, but it also caused pain of mucosa and distal abutment damage,then these results impacts on the effect of the quality of removable partial denture. Thus, reduce the amount of denture saddle tissueward motion and the damage of distal abutment is difficult on repairing tooth-tissue-borne RPD. The altered cast procedure is a method of functional impression. On this basis, the tissueward motion of distal-extension would be decrease when the denture under occlusal pressure. It can also improve the stability of denture. All this would be decreased lateral torsional rotational to the distal abutment teeth.Objection: According to investigate that the clinical result of mandibular distal edentulous removable partial denture which made by altered cast technique.Methods: Choose from 2014 to 2015 to our hospital for treatment of mandibular distal denture 4 patients. Including 3 cases of male, 1 female, the oldest patient aged 61, the youngest is 48 years old. There are three cases of mandibular bilateral free missing, one case of a unilateral free missing. Learn more about the patient complained, through clinical examination, oral preparations before treatment and then communicating with the patient’s treatment plan. All of their RPD use altered cast technique making impression. During the follow-up of 3to8 months,evaluate their clinical effect of repair by denture base upward motion, soft tissue tenderness, denture edges stretch.Results: Four patients with wear prosthesis retention is good, stable and no significant upward motion, edge stretching well, no soft tissue tenderness. After 5 days, 1 patient had tenderness on the left side organization. After modification the denture came back to normal. Four patients were satisfied with the denture effect.Conclusion: Four patients with early wear prosthesis retention is perfect, stable and no significant upward motion, edge stretching well, no soft tissue tenderness. After 5 days, one patient had tenderness on the left side organization, after grinding the wearing comfort. Four patients were satisfied with the denture effect.
