The Design Features And Clinical Case Report On Removable Partial Dentures Of Distal Edentulous Area

Background: It is common for people at various age groups to form dentition defects for a series of reasons,such as caries,periodontal disease,trauma and unreasonable usage of teeth.As a leading organ for developing masticatory function,back teeth have higher usage rate.Therefore,the lesion probability for the aged is larger.According to the classification of Kennedy,defect of dentition can be divided into four categories: the first one and second one are free-end deficiency,which is common clinically.Social development makes people's living standard improved.With the increase of life quality,people pay more attention to the masticatry function.Distal edentulous area is difficult to be repaired clinically.The repair of distal edentulous area mainly applies active repair.As a category of partial denture,it is widely applied and is simple and feasible relative to implant restoration and fixed-removable prostheses.Moreover,it also has smaller destructive effect to soft or hard tissues in oral cavity and is relatively economical and practical.The situation of distal edentulous area varies with each individual,such as conditions of remaining natural teeth,situation of periodontal supporting tissues,and absorptive degree of alveolar crest,and so on,which can generate a large influence on remediation effects of removable partial dentures.According to different situations,it is extremely important to design suitable removable partial dentures of individuals.The key of successful removable dentures is to design reasonably.Moreover,it is also important to do tooth preparation,impression taking and processing production and so on.Purposes: To discuss clinical design scheme and effect evaluation on repairing distal edentulous area of removable partial dentures.Materials and methods: 8 patients of Kennedy I and II of tooth loss who saw a doctor in our hospital from March 2014 to March 2016,including 3 male patients and 5 female patients.According to patients' chief complaint and detailed oral inspection,the design of casting removable partial dentures was confirmed.Patients signed the informed consent.According to a repairing therapeutic schedule,selective pressure impression of tooth preparation was prepared.Dentures were manufactured conventionally.After 3-12 months,patients were paid a return visit.According to clinical inspection and patients' satisfaction degree,remediation effects were evaluated.Results: 8 patients were casted supporting removable partial dentures to repair.All of them repaired completeness of dental arch and facial appearance and beauty.6 patients had good adaptation after wearing dentures,and retention,supporting and stable effects of dentures were excellent.After 1-2-week further consultation,they have acquired satisfactory repairing effects.2 patients had more number of tooth loss,heavy absorptive degree of alveolar crest,and excellent retention effects of dentures,but supporting and stable effects were worse.These two patients have rechecked for many times.After repeated concoction and partial buffer,satisfactory repair effects were finally obtained.Conclusions: Removable partial dentures are applied to repair distal edentulous area.It is necessary to do detailed inspection on patients' oral situation,formulate excellent retention,supporting and stabilization reasonably,and design a denture scheme suitable for patients' conditions.This not only can recover and improve patients' oral physiological function,ensure success rate of repairing,and improve patients' satisfaction.In the repairing process of removable partial dentures,it is vital to design dentures.Only to formulate excellent retention,stabilization and supporting roles reasonably,base on denture design suitable for patients' conditions,and perfect atraumatic restorative treatment can it recovers and improves oral physiological functions,ensure repair success rate,and improve patients' satisfaction.
