Background:Partially edentulous arches means that different parts in maxillary or mandible dentition have a variable number of teeth losses,but dentition also has a variable number of natural teeth.It will impact mastication,pronunciation,cabochon and appearance of people directly and reduce living quality and mental happiness of people.There are so many ways to repair partially edentulous arches.At present,the application of removable partial denture is the widest,especially for mandibular distal edentulous,it even gives priority to repair removable partial denture.Mandibular distal edentulous generally adopts tooth-tissue-borne RPD to repair.When tooth-tissue-borne RPD resultant force,average mobile range of abutment periodontal membrane is0.03 mm,while mobile range of distal edentulous alveolar mucosa is 0.14-0.35 mm.The average is 0.2mm.The formative difference between them can reach 5-10 times,and such the difference will make distal denture saddle sunk obviously under the functional state of denture,make terminal abutment damaged for far twisting force,and make alveolar mucosa in the distal edentulous area generate pain for pressure or cause absorption of alveolar crest for excessive pressure.Clinically,when using tooth-tissur-borne RPD to repair mandibular,selective pressure impression is prepared to avoid and remit the above-mentioned occurrence.Selective pressure impression refers to acquire impression,when soft or hard tissues in oral cavity assume certain pressure,which can get impression of which alveolar mucosa and alveolar crest in distal edentulous area are kept in functional status is prepared,while acquiring the anatomic state of the rest of teeth.The research plans to adopt the widest clinical application of red compound-alginate secondary impression technique and altered cast technique to repair mandibular distal edentulous.Moreover,the research makes a comparisonbetween clinical effects of repairing mandibular distal edentulous between two different selective pressure impressions through adaptive inspection and patient satisfaction of wearing denture for 3 months,so as to guide future work.Purposes: To observe and make a comparison with clinical effects of mandibular distal edentulous between two different selective pressure impressions,so as to guide clinical operation.Methods: 40 patients(17 males and 23 females with the average age of 67.2 years old)with mandibular distal edentulous diagnosed in stomatological hospital affiliated to Dalian Medecal University from August 2014 to December 2015 were selected.These patients were divided into A group and B group at random.A group adopted red compound-alginate secondary impression technique to make impression;group B applied altered cast technique to make impression.Both of them were general designed and formulated removable partial denture.They were followed up for 3-10 months.Through clinical inspection and patient satisfaction survey,clinical effects of final RPD of A group and B group were compared.Results: A group and B group had advantages in denture edge extension,denture and mucosa closing degree,reducing mucosal pain and 3-month satisfaction wearing denture;and B group shows outstanding performance in reducing mucosal pain.Conclusions: The removable partial denture made by using two-step impression technique with red compound and altered cast technique repairs mandibular distal edentulous,reduces times of follow-up,and improves near and far-date effects of denture repair.It has the highest patient satisfaction.
