| Objective: To evaluate the clinical effect of digital surgery combined with 3D printing technology in the immediate repair of mandibular defect and the later repair of dentition defect.Methods: From July 2018 to June 2020,8 patients with mandible tumor or defect after tumor operation in Xuzhou central hospital were selected.All patients were examined by spiral CT and CT angiography(CTA)of lower limbs before operation,and the operation process was simulated by digital surgical PROPLAN 3.0 medical processing software and 3D printing technology,including osteotomy range design and defect image reconstruction of mandibular lesions;all kinds of osteotomy,shaping and reduction guide plates were printed after the simulation operation effect was satisfied,and the 1:1printing model was used Pre bent reconstruction titanium plate,etc.During the operation,according to the osteotomy guide plate,shaping guide plate to complete the mandibular resection,fibular muscle flap preparation,shaping and immediate repair of mandibular defect,etc.,after the operation,the implant guide plate was designed by digital software to complete the repair of secondary dentition defect.The follow-up included facial satisfaction,mouth opening and occlusal relationship.The osteogenic effect and joint function were evaluated by ECT and joint MRI.Results: 8 cases of patients with preoperative aided design of digital guide in the application of intraoperative get accurate,think in terms of patients: effectively save the operation time,reduce the postoperative complication,shorten in-hospital time,save the economic cost,postoperative 1 month,3 months,6 months and 1 year review: type plane symmetry is good,the mouth and occlusal all normal,joint function is stable.From the perspective of doctors: the digital preoperative rehearsal of the surgical process has made the communication between patients more intuitive and clear,and has been applied to teaching and scientific research more intuitively,thus reducing the occurrence of complications and enhancing doctors’ surgical confidence.Two patients successfully repaired the dentition defect with the assistance of the secondary implant guide plate.Conclusion: the application effect of computer-aided surgery technology in the immediate repair of mandibular defect and the later repair of dentition defect is clear,which can better improve the functional requirements of facial,occlusion and mastication of patients with mandibular defect repair,obtain the satisfaction of patients,and meet the operation requirements of personalized functional surgery. |
