Objective Compare the laparoscopic hepatectomy with the open hepatectomy in cohort studies of colorectal liver metastasis(CLM) by retrospective analysis, to explore the application of laparoscopic hepatectomy in the treatment of colorectal liver metastasis.Methods20cases of colorectal cancer liver metastasis cases were prospectively collected from January1st,2011to December31th,2014in Sir Run Run Shaw Hospital affiliated to Zhejiang University. Comparing operation time, intraoperative blood loss,transfusion rate, postoperative hospitalization days, postoperative liver function, postoperative complications, and long-term follow-up results.Results Compared with open hepatectomy(OH) group, the average operation time of laparoscopic hepatectomy (LH) group with247.22minutes (50-360), was slightly higher than that of the OH group, which was205.63(110-360) minutes; In terms of intraoperative blood loss,the two groups were305.0(50-800) ml and693.8(50-2000) ml, the average hospitalization days of LH group was10.2(5-23) days, significantly shorter than that of OH group with14.50(3-59) days; both were no significant difference. Compare of the liver function in the thrid day after surgery, total bilirubin of LH group was13.50±5.76umol/L, significantly lower than the that of OH group with26.28±10.42umol/L, P value was0.017, with significant difference. No significant diferences was found in recurrence rate or overall survival during the average time of follow-up in the LH and OH group were31(6-45) months and23.6(7-40) months.Conclusion Laparoscopic hepatectomy has a mild advantage in terms of recovery of postoperative liver function than that of open hepatectomy in the treatment of colorectal liver metastasis. The average hospitalization days and the average operation time of LH group are less than that of OH group without statistical significance which needs more researches to comfirm. No significant diferences was found in recurrence rate or overall survival between the LH and OH with CLM.
