| Objective:This study compares the short-term efficacy and hospitalization costs between open hepatectomy(OH)and laparoscopic hepatectomy(LH),so as to provide a reference for surgeons in the choice of surgical methods.Methods:From January 1,2012 to October 30,2018,we collected data of 98 patients with liver disease underwent laparoscopic and open resection in Peking University Shenzhen Hospital.There were 21 cases of benign diseases and 17 cases of malignant diseases in LH group,28 cases of benign diseases and 32 cases of malignant diseases in OH group.The data of the two groups were compared in operation time,intraoperative bleeding,hepatic hilar occlusion,postoperative out of bed time,postoperative resumption of eating time,postoperative continuous use of painkillers,postoperative discharge time,whether complications occurred,postoperative complications,postoperative painkillers,postoperative discharge time,postoperative complications,postoperative complications.The changes of liver function(ALT/ ALB/TIBL)and the difference of hospitalization expenses on the first day.To evaluate the difference in short-term efficacy and cost between laparoscopic hepatectomy and open hepatectomy.Results:Compared with the LH group,the OH group,The LH group is better in operative time(271.42 ±82.09 VS 201.13 ±72.83 min),intraoperative bleeding(1138.33 ±1021.48 VS 497.37 ±651.92 ml),hilar obstruction rate(41.7% VS 21.1%),postoperative out of bed time(5.07 ±1.49 VS 2.21 ±0.74 days),postoperative resumption ofeating time(3.73 ±1.49 VS,2.19 ±0.82 days)and discharge from hospital time(14.87 ±6.02 VS,10.47 ±7.00 days).There was significant difference between the two groups(P < 0.05).There was no significant difference in the duration of continuous use of painkillers(3.83 ±1.61 VS 3.21 ±1.46 days),the incidence of complications(13.3% VS 10.5%),the change of liver function one day after operation ALT(154.74 ±220.84 VS 110.95 ±80.09 U / L),ALB(-10.92 ±6.87 VS-9.56 ±5.32 g /L)and TBIL(6.68 ±26.61 VS 4.77 ±9.30 umol/L).The average cost was lower than that of OH group(62439.52 ±24994.47 VS 57883.94 ±20792.79 yuan),but there was no significant difference between the two groups(P > 0.05).Conclusion:Under the condition of strict implementation of indications,LH has an advantage over OH in short-term postoperative efficacy,and does not increase the cost burden.Clinical surgeons need to take a comprehensive consideration to choose the appropriate surgical method. |
