Clinical Research On The Intervention In Post-stroke Depression Of Peiyuan Ditan Kaiyu Decoction

Objective: Evaluate the clinical curative effect of Peiyuan Ditan Kaiyu Decoction on early prevention of post stroke depression and explore its possible mechanism.Methods: Divide 106 patients into control group and treatment group with 53 cases in each group by the stochastic method. The control group with conventional treatment, the treatment group with conventional treatment and the Peiyuan Ditan Kaiyu Decoction, the course of treatment is 28 days. Score the patients in the two groups and assess the clinical efficacy before treatment, when the patients were treated for 14 days and 28 days with HAMD, ADL a nd NIHSS. Detect the hs-CRP levels of serum before and after treatment.Results:(1) In the clinical observation of the study each group lost 3 cases, there were 50 cases in each group. The gender, age, stroke position, past history score, HAMD score, NIHSS score, ADL score, serum hs-CRP content of the patients before treatment in two groups had no significant difference(P > 0.05), and comparable.(2) Comparison of HAMD score before and after treatment: the treatment group and the control group with the change of time, the HAMD score are increased, compared with those before treatment there were differences(P < 0.05).The treatment group and the control group by variance analysis the difference was significant(P < 0.05). Compared with the control group, the integral of treatment group rises slowly. After the intervention of Peiyuan Ditan Kaiyu Decoction, compared with the control group, the degree of depression of treatment group is light.(3) The NIHSS scores before and after treatment were compared between the two groups: the treatment group and the control group with the change of time, NIHSS score decreased, by analysis of variance the difference was significant(P < 0.05), suggesting that improved in different degrees after treatment of two groups of patients; The difference of the comparison between the treatment group and the control group was significant(P < 0.05), suggesting that the treatment group improved better than the control group.(4) The ADL score before and after treatment were compared between the two groups: the treatment group and the control group with the change of time the ADL integral were increased, by variance analysis, the difference was significant(P < 0.05), suggesting that there were differences compared with before treatment; the differences compared between the treatment group and the control group were significant(P < 0.05), suggesting that the improvement of the ability of daily life in the treatment group is significantly better than the control group.(5) The comparison of serum hs-CRP content in two groups: compared with after treatment by the t test the differences were significant(P < 0.05), the content of hs-CRP decreased obviously; compared between two groups after treatment the differences were significant(P < 0.05), suggesting that the content of hsCRP of the treatment group reduce more than the control group.(6) The incidence of PSD in the two groups after treatment: the incidence of PSD in treatment group is 40%, in the control group is 18%, byχ2 test, the differences in two groups before and after treatment were significant(P < 0.05), suggesting that the incidence of PSD in the treatment group was significantly lower than the control group.(7) The comparison of NIHSS changes in the two groups after treatment: the effective rate of treatment group was 90%, the control group was 72%, byχ2 test, the differences were significant(P < 0.05), suggesting that the improvement of nerve defect in treatment group was better than the control group.Conclusion:1 The early application of Peiyuan Ditan Kaiyu Decoction in patients with acute cerebral infarction can significantly reduce the incidence of PSD, improve the nerve function defect, improve the quality of life; 2 The intervention mechanism of Peiyuan Ditan Kaiyu Decoction in post stroke depression may be related to anti inflammation. 3 The results of this study suggests that Peiyuan Ditan Kaiyu is the important therapeutic principles to post stroke depression.4 Peiyuan Ditan Kaiyu Decoction is safe in clinical application, and have no obvious adverse reaction.
