Objective: Patients with severe acute pancreatitis(SAP) is often associated with abdominal hypertension(IAH), but small number of patients attack abdominal compartment syndrome(ACS) due to illness varies. SAP patients with low probability of ACS, but very high mortality rate, and its course of development of a special pathophysiology and clinical features. Clinical departments in dealing with the patient’s needs related to the merger of SAP IAH or ACS identify and take corresponding treatment strategies. SAP consolidation in patients with ACS symptoms and rapid progression of the disease risks of aggressive treatment measures should be taken. SAP merger ACS patients should be prudent in dealing with surgery, but tardive ACS can remove causes of the disease while abdominal decompression, which usually achieve better results. These factors prompt us to fully understand and grasp the cases encountered in clinical work, carefully analyzed and summarized in order to better serve the clinic. Here we summarize relevant experience through a systematic review analyzed 16 cases of Dalian Friendship Hospital with severe acute pancreatitis(SAP) with abdominal compartment syndrome(ACS) after clinical treatment. Methods: On the basis of SAP common treatment to combine relevant experience and to improve the monitoring IAP; Further emphasizes rigorous testing within the dynamic abdominal pressure(pressure within the bladder); Optimized liquid composition, and actively implement resuscitation; Hemofiltration, to improve abdominal wall compliance; Abdominal decompression, early enteral nutrition and other therapeutic measures. Results: After active treatment, 16 cases of SAP merger ACS patients, 4 patients died, the mortality rate was 25%. Deaths were renal failure, in which one cases with respiratory failure, and 1 patient with heart failure. The above patients were all treated with this therapy, and summed up treatment strategy for SAP merger ACS patients. Conclusion: The key for SAP merger ACS treatment is early diagnosis, rational and effective treatment.
