Objective To describe the clinical characteristics of AP, explore the hazards of AP complicated with I AH, and study the pertinence of treatment and prognosis.Methods Data of hospitalized patients of Wuhan Union Hospital from Jan1st,2008to Jun30,2013were retrospectively analyzed. Study the age, gender and other clinical characteristics of AP.Results1)Total hospitalized210cases diagnosed as acute pancreatitis from Union hospital,129male cases(61.4%) and81female cases(38.6%);hospitalized within24h from abdominal pain were60cases,24h to48h were92cases,and48h to72h were58cases;101cases were biliary tract disease,28cases were because of alcohol,30cases were hyperlipidaemia.2)IAP single factor pertinence:BMI, correlation coefficient is0.193, sig.=0.005; WBC, correlation coefficient is0.217, sig.=0.002; APACHE Ⅱ score, correlation coefficient is0.487,sig.=0.000; Balthazar CT score, correlation coefficient is0.524, sig.=0.000; ALB, correlation coefficient is-0.264, sig.=0.000;72h fluid net gain, correlation coefficient is0.222, sig.=0.000.3)IAP multiple factor pertinence.-APACHE Ⅱ score, partial correlation coefficient is0.442, sig.=0.000; Balthazar CT score, partial correlation coefficient is1.305, sig.=0.000; ALB,partial correlation coefficient is-0.225, sig.=0.000;72h fluid net gain, partial correlation coefficient is0.322, sig.=0.014. Regression model is IAH risk index=11.335+1.305* Balthazar CT score+0.442*APACHE II score-0.255*ALB+0.322*72h fluid net gain.4)210cases, ACS cases were69, IAP is25.41±3.98mmHg, APP is58.41±6.29mmHg;non-ACS cases were141cases,IAP is18.87±5.40mmHg,.APP is67.27±4.33mmHg5)Regression analysis the pertinence of treatment and prognosis in ACS group:minimally invasive therapy or not, partial correlation coefficient is-1.211, sig.=0.000; the days from ACS to fluid negative balance, partial correlation coefficient is0.635, sig.=0.000. Regression model is ACS prognosis index=5.23-1.211*minimally invasive therapy or not+0.635*the days from ACS to fluid negative balance.Conclusion IAP has no significant correlation in AP patients with gender, age and pathogenesis, etc. IAP has significant correlation in AP patients with APACHE II score, Balthazar CT score, ALB and72h fluid net gain in multiple factor regression analysis. Regression model is IAH risk index=11.335+1.305*Balthazar CT score+0.442*APACHE II score-0.255*ALB+0.322*72h fluid net gain. The IAP of ACS group were significant higher than non-ACS group. minimally invasive therapy and making fluid into negetive balance are benifit to the prognosis of ACS in AP patients.
