Background：In one’s life,old age is the process of body function overall recession,resistanceand immunity decline,leading to the risk of suffering from different diseases,has ledto the ways and means for the elderly health care is more complex,medical costs aremore expensive,need more material and financial resources.Our country is theworld’smost populations nation,accordingly,the number of elderly people in China isalso one of the world’s first,carried out the2010six national census data show that infact in our country has stepped into an aging society.The particularity of the oldenvironment,we need to pay more attention to and care for this group of medicalproblems,released in2006,the Chinese govement white paper on “China agingdevelopment”show the attitude of the Chinese government on aging,also reveal theseriousness of the current population aging in our country.Arranged in reforming theold institutions of our country was born-growth-death pension system at the sametime,also in the reform of traditional medical insurance system.Because of thelimitation of state of the current medical insurance system in our country presents thecharacteristics of “low level,wide coverage”,also because of this,the majority of theelderly to enjoy medical is low level.Objective：Based on the medical insurance system in our country at present stagedevelopment situation and the trend of population aging,based on the composition ofChangchun city elderly health insurance and realistic demand,the developmentpresent situation,the analysis of existing problems,puts forward to perfect theChangchun city elderly health insurance countermeasures,Changchun city for betterdevelopment of the elderly health to insurance to provide theoretical guidance and countermeasures for reference.Methods：Changchun,this study collected the related reports,the system of the basicmedical insurance policy;Refer to the Jilin province in1953-2010populationcensus,China health statistics yearbook and the statistics bulletin of the nationaleconomy and social development of JiLin province.To Changchun city hospitalsaccording to levels(provincial,municipal,district-level)layers,each level randormly ahospital,the three hospital inn June2013-June2014patients during hospitalizationsampling investigation and statistics analysis,in order to understand the basicsituating of Changchun city sampling investigation and statistics analysis,in order tounderstand the basic situation of Changchun city elderly health insurance,it discussesthe current situation of Changchun city elderly health insurance,the challenge,andadvances some countermeasures and suggestions.Results：(1)Through to the Changchun city in1953-2010,comparing the status of thepopulation age structure and population aging found that Changchun city populationaging the following characteristics:the rapid growth of the elderly population agestructure aging,population aging trend is obvious.(2)Through the treatment of medical treatment insurance payment of thecomparative analysis found that town retiree delimit individual account in theamount of worker medical treatment ratio is higher than on-the-job worker.In thehospital,at the same level medical institutions for medical treatment,patients with thesame care type,on-the-job personnel and retirees with reference to the samefubiao.Different types of health insurance,the city health starting line aboveprovincial health care;The higher the hospital level,fubiao is higher;At all levels ofmedical institutions as a whole fund pays proportion of retirees were higher than thatof on-the-job worker,the higher the level of hospital,the lower the proportion ofreimbursement. The fubiao since in hospitalization medical insurance for urban residents,thehigher the level of medical institutions,the higher the starting;The category ofdifferent of personnel,no fubiao set specifically foe the elderly.On hospitalreimbursement ratio,the higher the level of medical institutions,the lower thereimbursement ratio.Also,for elderly patients with no clear policy support.(3) On the elderly chronic disease prevalence in the elderly and through thecomparative analysis of found that elderly people with more than two and two aresignificantly higher than proportion of the chronic disease of the elderly,tewo kindsof disease:44.67%to37.80%of more than two kinds of disease.In the elderly andelderly health care costs:found in the comparative analysis of the elderly health costsmean higher than that of non elderly patients,average costs are:9069.11yuan and8453.28yuan.In the analysis of the overall pay and pay scale:found in older people asa whole pay respectively,and the payment proportion were44.20%,55.57%respectively.Pay in cash and payment ratio analysis found that:the average cash costof old people and pay higher proportion of older people,the average cost is3425.19yuan and2501.45yuan respectively,payment proportion were42.66%,37.29%respectively.Through the analysis of the elderly and the elderly health care found:60-69agegroup has the highest average costs,followed by80years of age or older group,thelowest in the70-79age group,the mean are:9181.45yuan,8943.33yuan and4867.51yuan.For the elderly of different age group as a whole pay and pay scale analysisfound that:as a whole pay mean and pay for the highest percentage of60-69agegroup,followed by80years of age or older age group,the minimum is70-79agegroup,average cost is4479.5yuan,3919.05yuan and2219.53yuan respectively,payrate were54.95%,48.60%and44.31%respectively.In different age group for theelderly in cash payment and payment and payment for the highest percentage of80years of age or older age group,followed by70-79age group,the lowest in the60-69age group,the average cost of3649.74yuan,2944.44yuan and1732.13yuarespectively,and payment proportion were49.28%,39.33%and33.34%respectively.
